Ozempic ( Semaglutide ) and diabetic retinopathy (DR)

DR is a microvascular complication of diabetes that can cause visual impairment and blindness. Long term glucose control significantly reduces the development of DR.

Paradoxically, intensive treatment of high glucose and rapid reduction in HBA1c in a short period of time were associated with a temporary worsening of DR in the first 3-36 months. This was noted in Ozempic use as well as other glucose lowering treatments such as Insulin or bariatric surgery.

If you are diabetic, when choosing Semaglutide as an adjuvant treatment for glucose control, it is recommended to visit your ophthalmologist prior to initiation of the treatment and regularly afterwards.

What is NAION?

Non-arteritic Anterior Ischemic Optic Neuropathy (NAION): it results form an obstruction of the blood vessels feeding the optic nerve thus causing an infarct.

Incidence is between 2.3-10.2/100000 in the US, mostly age between 57-65 years.

Causes: No clear cause was confirmed, but might be associated with Sleep Apnea, high blood pressure, some anatomical variations in the optic nerve and some medications ( including Ozempic )

Symptoms: painless loss of vision mostly noted upon awakening. Often visual loss noted in inferior visual field.

Could Ozempic cause blindness?

A new study published in JAMA Ophthalmology journal in July 2024 suggests an association between Ozempic and a condition called Non-arteritis Ischemic Optic Neuropathy (NAION). Though a rare disease, NAION can potentially cause blindness. Risk of developing NAION was found to be 4.28 times higher in diabetics and 7.64 times higher in obese patients. Despite the associated risk, the design of the study doesn’t imply a causal relationship between the two. Larger prospective randomized studies or post-market analyses would be needed in the future to confirm the thesis.
