Cellulitis is an infection of the deep dermis and subcutaneous tissue. It causes a localised area of red, painful, swollen skin that is usually well demarcated. The most common site of infection is the lower limb.

Most common cause: Streptococcus pyogenes
Second most common cause: Staphylococcus aureus

Severity graded using Eron Classification 1-4:
• Grade 2 cellulitis or higher is classified as severe and hospital admission should be considered
• Cellulitis of the face is also considered severe

First-line treatment of non-severe cellulitis (Grade I & not on the face) is with Flucloxacillin 0.5-1.0g qds for 7 days total.

Nasopharyngeal carcinoma is a rare tumor arising from the epithelium of the nasopharynx. It accounts for approximately 1% of all childhood malignancies. It is commonly associated with EBV, where virally infected cells undergo malignant transformation.

Radiotherapy is the mainstay of treatment, with chemotherapy being used in advanced cases. Surgical treatment is often limited to a biopsy for tissue diagnosis. Nearly all tumors are unresectable at diagnosis because of their location.

The spleen plays an important role in filtering blood by removing old or damaged cells and debris, as well as helping the body fight infections. The spleen also stores white blood cells and platelets. An enlarged spleen is characterised as one that is larger than 12 cm (approximately 5 inches) in length or over 400 grams (approximately 14 ounces) in weight.

Massive splenomegaly refers to significant enlargement of the spleen, usually >20 cm in length or over 1 kg (approximately 2.2 lbs) in weight. In such cases, the spleen may cross the midline of the abdomen and extends toward the RLQ. Diseases that can cause such massive splenomegaly include various cancers, such as CML, myelofibrosis and splenic marginal zone lymphoma, as well as certain infections, such as malaria.

Multiple myeloma is a malignancy of plasma cells in which monoclonal plasma cells proliferate in bone marrow resulting in:
• an overabundance of monoclonal paraprotein (M protein)
• destruction of bone
• displacement of other hematopoietic cell lines

Multiple myeloma can range from asymptomatic to severely symptomatic with complications requiring emergent treatment. The key features of the condition can be remembered with the mnemonic CRAB.
