Is Menopause an illness or a disease?

Menopause is a physiological phase of life. It is neither an illness nor a disease. Are we over-medicalizing menopause nowadays by talking so much about Hormone Therapy? Well, that is also not true.

Some women can go through their menopause in a very smooth and easy way. But we really don't know what is hiding behind. For some women, menopause can really be a struggle. It could be hot flashes, night sweats, fatigue, and tiredness.

I get so many women who complain about not being able to sleep because of the fatigue, the body pain, the muscle, and the joint pain. So, there is help and support available in case you are struggling. You don't need to struggle through all these symptoms.

Menopause is a phase of transition, but it can last for 3 to 10 years. And remember — you're not getting an award for struggling with all these symptoms.

Perimenopause
Perimenopause is the stage of transition before menopause. During this period, estrogen (estradiol) and progesterone levels can swing dramatically — even from one hour to the next. This phase may last only a few months, or in some women, up to a decade. These hormonal shifts often bring symptoms such as cycle changes, hot flashes, disturbed sleep, mood fluctuations, and fatigue.

Menopause
Menopause is confirmed once a woman has gone 12 consecutive months without a period. But it is not simply the end of menstruation — it reflects an ongoing hormonal decline. Levels of estradiol, progesterone, and testosterone produced by the ovaries (and to some extent by other organs) fall considerably.

Because these hormones are vital messengers that act on every cell, their reduction impacts many systems of the body, including the brain, bones, heart, metabolism, and emotional well-being. Menopause should be seen as a permanent hormone deficiency, rather than a single moment in time.

Did you know your body makes three different types of estrogen—and they each play unique roles?

🔹 Estrone (E1) – Produced in fat tissue & adrenal glands. It’s the only estrogen left after menopause. But in excess, it can increase the risk of DNA changes that may lead to cancer.
🔹 Estradiol (E2) – Made in the ovaries. This is the most powerful estrogen, vital for heart, bones, brain & energy. It’s the dominant estrogen during reproductive years.
🔹 Estriol (E3) – Mainly from the placenta (also liver & breasts). Known as the “gentle” estrogen, supportive for skin and tissues, especially during pregnancy.

Vaginal Estrogens are safe

Vaginal Estrogens are safe. Even if you have had the other contraindications like a history of Breast Cancer, va**nal estrogens can be used because they are not absorbed systematically.

There are different formulations like va**nal creams, estriol creams (0.5 to 1 mg), which can be used. First, we start by using them every single day for two weeks, then on alternate days for the next two weeks, and then 2 or 3 times per week. Every week a Consistent use is important.

Why does this matter? Because a dry va**na or Genitourinary Syndrome of Menopause means that the skin or the lining of the va**na is becoming very thin, making it prone not only to dryness but also to frequent urinary tract infections.

So, you can start using va**nal estrogens even if you have contraindications. It is absolutely safe, and consistent use will help improve symptoms—usually in 4 to 6 weeks.
