📌 راډيولوژيکي راپور
نن ورځ د يو ناروغ لپاره د Intravenous Pyelography (IVP) معاينات ترسره شول. د معايناتو پر بنسټ، په دواړو ګردو کې هایډرونفروسیس مشاهده شو.
سره له دې، د کنټراسټ مادې اخيستل او خارجېدل د دواړو ګردو له خوا په مناسب ډول ترسره شول، چې د ګردو د فعاليت (Function) شتون څرګندوي.
🔹 دواړه ګردې فعالې وې
🔹 کنټراسټ اخيستل او خارجېدل نورمال وو
🔹 هایډرونفروسیس موجود و، چې د لا زياتو کلينيکي ارزونو او تعقيب اړتیا لري
📍 پایله: سره له دې چې اناتوميکي بدلون (Hydronephrosis) شتون لري، خو د ګردو فعاله وظيفه يو مثبت نښه ده.

IMPRESSION:

· Visualized lung fields show few soft tissue attenuation nodules, largest in right lung base measuring about 28 x 25mm – likely suggestive of metastatic etiology.

· Heterogenous mass measuring about 17.2 x 13.6 x 19.8cm is seen in right upper quadrant involving almost whole of the right kidney and shows indistinct interface with adjacent liver. The right adrenal gland is also not separately visualized. The right renal vein and IVC are not distinctly visualized raising possibility of involvement/ thrombosis. It is encasing the aorta across the mid-line and shows heterogenous post contrast enhancement and areas of internal necrosis. It is also abutting the gall blader. Posteriorly it is reaching the posterior abdominal wall with irregular fat strandings. Few nodular deposits are also noted in the abdominal cavity inferior to this mass lesion, the largest on left side measuring about 46 x 49.7mm and largest on right side measures about 41 x 40mm. Neoplastic etiology – RCC likely.

· A small umbilical hernia is seen, and few enhancing nodules are also noted within it, largest measuring about 16 x 16mm.

OPINION:​
Suggestive of malignant neoplastic pancreatic mass with hepatic metastatic deposit, moderate malignant ascites, peritoneal nodules and omental caking-suggestive of stage IV CA pancreas
Minimal pleural effusion left
Right pulmonary artery embolism
ADV:Histopathological correlation

impression
A well-defined rounded intramural extra medullary area is noted posterior to body of DV4. It measures about 9.6 x 13.4 x 17mm (AP x TR x CC) dimension. It is hypointense on T2 W images and slightly hyperintense on T1 W images and shows significant post contrast enhancement. It is causing compression of the cord in this region. evident as contrast column cutoff in MR myelogram.
Possible differentials would include a meningioma and nerve sheath tumour. Clinical and histopathological correlation is recommended.

IMPRESSION:
Visualized chest shows a hypodense area in the left atrium measuring about 26 x 28mm-may represent thrombus. Echocardiography is recommended for further evaluation
· Cardiac enlargement is seen with cardiac ratio>60%.
Spleen is enlarged measuring about 15.4cm and shows heterogenous confluent hypodense areas in the periphery of spleen as well as in the central part-likely representing splenic infarcts.
·A cyst measuring about 20 x 22 mm is seen in interpolar region of left kidney.
· Scoliosis of the lumbar spine is seen convex to left side. degenerative changes are noted in the spine.
Prostate is mildly enlarged measuring about 3.9 x 4.6 x 4.0 cm and weighs about 41 grams.
