IMPRESSION:
· Visualized lung fields show few soft tissue attenuation nodules, largest in right lung base measuring about 28 x 25mm – likely suggestive of metastatic etiology.
· Heterogenous mass measuring about 17.2 x 13.6 x 19.8cm is seen in right upper quadrant involving almost whole of the right kidney and shows indistinct interface with adjacent liver. The right adrenal gland is also not separately visualized. The right renal vein and IVC are not distinctly visualized raising possibility of involvement/ thrombosis. It is encasing the aorta across the mid-line and shows heterogenous post contrast enhancement and areas of internal necrosis. It is also abutting the gall blader. Posteriorly it is reaching the posterior abdominal wall with irregular fat strandings. Few nodular deposits are also noted in the abdominal cavity inferior to this mass lesion, the largest on left side measuring about 46 x 49.7mm and largest on right side measures about 41 x 40mm. Neoplastic etiology – RCC likely.
· A small umbilical hernia is seen, and few enhancing nodules are also noted within it, largest measuring about 16 x 16mm.
