Dx for this case

An 18-year-old woman presents to the emergency department complaining of chest discomfort. On physical examination, she is mildly tachypneic and tachycardic, with a fever of 101° F. Her past medical history is significant for a seizure disorder. Her chest x-ray is above.
What is the most likely diagnosis?
1) Fungal pneumonia
2) Squamous cell carcinoma of the lung
3) Lung abscess
4) Pulmonary embolus

*During normal pregnancy, patients develop a relative anemia due to increases in plasma volume and (but to lesser degree) erythrocyte mass.
*Mean arterial blood pressure slightly decreases due to reduced systemic vascular resistance, but heart rate and cardiac output increase (cardiac output increases by nearly 40% above the pre pregnancy level, and by 80% during delivery owing to increased heart rate and blood pressure).
:
• mild peripheral edema usually occur
• mild dyspnea or exertional dyspnea can occur
• BP typically decreases by about 10 mm Hg
• HR increases by 20-30%, but not tachycardia
• premature atrial or ventricular beats may be seen; but not AF, AFL, or VT
• basal systolic murmur grade 1/6 to 2/6 is present in 80% and/or S3, but diastolic murmur, systolic murmur grade 3/6 or more, or S4 is pathologic.
