A 25 yrs old male presented in Emergency with history of RTA and complaint from abdomen and chet pain.
Work up:-
Physical exam:- Ab pain and normal vital signs
US:- free fluid in peritoneal cavity with pleural effusion
CT scn:- Grade 3 liver lacerations in 6s-5s+Hemoperitonium+ mild pleural effusion
Plan:- laparatomy + 1200cc 🩸 in peritoneal cavity
Omental pack and hemostasis
Now:- patient is stable

A very sad case 38yrs olda female presented in emergency with abdominal pain,distension and since three months but become sever in last six days which caused to attend hospital after
History- Umbilical hernioplasty
Physical exam:-Distend, rigid and board like abdomen
Work up:-
1-Ab x-ray pneumoperitonium
2-CBC:-leukocytosis
3-RFT : normal
Finallay:- GI perforation
Laparotomy:- Mesenteric Tumor which incase Ileum and made a band with abdominal wall cause perforation
Procedure:-Ileustomy +Biopsy

A 46yrs old female with history of mastalgia and lumpiness in left upper quadrant of left breast since six month:
1:-Physical exam
2:-Ultrasound
3:-CT scan
4:-FNAC
Final Diagnose:- Invasive ductal carcinoma with regional lymph node metastasis

A 50yrs old female presented with bilateral flank pain and oligouria with gross hematuria
Work up:-
:-RFT: Creatinine=6
:-US:- Bilateral sever hydronephrosis and UB mass
:-CT without contrast:- UB completely occupied with an irregular outgrowth mass encase anterior vaginal wall with regional lymphadenopathy.
Dx:-urethelial carcinoma (Transitional)
:-Bilateral nephrostom applied until creatinine 1.7

Plane:-Radical cystectomy+Radical hysterectomy and Urostomy was done
Now:- patient is stable and after recovery chemotherapy will start.

A 35 years old male present with complaint of abdominal pain specially left flank, weight loss and anorexia.
Work up:-
US:- left suprarenal large mass
CBC:- HB 8
Contrast CT:- A large well define multi-loculated mass size ( 15x17cm) in suprarenal and mass effect on upper pole of left kidney without any lymph nodes enlargement
Final plan:- Abdominal left subcostal incision Abdominal Acess and mass excision with Renal and spleen preservation.

Imaging:- CXR normal
eFAST positive
Dx:- spleen Rapture
Plane:- Dx laparotomy
Post op DX:- spleen grade. IV Rapture
Splenectomy

مریض که مدت ها از دردکمرشاکی بوده و نزدش psoas Abscess تشخیص شد وعملیات شد تشخیص بعد از عملیات یک واقعه نادر Retroperitoneal hydatidcyst or Psoas Muscle hydatid cyst بود که بخوبی عملیات گردید
