This is what we're building.
Care that holds together, without families having to hold it.
Most NDIS supports run in parallel. The Allied Health provider has their goals. The support worker has their schedule. They don't share information. The young person gets an hour of therapy, and the rest of the week looks nothing like it.
That gap falls on families to bridge. Relaying what was covered in the session. Reminding the support worker what strategies to use. Carrying information that should already be shared.
At GCC, Allied Health sets the direction and Support Workers follow those strategies in the everyday moments. When your young person is in the community, at home, at school, the approach stays consistent. Allied Health isn't just 1 hour a week. It's carried through every time we support them.
We're starting with Speech Therapy, with big plans to grow. The same connected model, with more Allied Health input carried through into daily life as the team expands.
