A Support Coordinator helps NDIS participants understand and make the most of their NDIS plan.

Their role is to build capacity, connect participants with the right services, and ensure supports are working effectively.

Key responsibilities include:

Understanding the plan:
Explaining funding categories and what supports can be used for.

Connecting with services:
Linking participants to community, mainstream, and NDIS providers that suit their goals and needs.

Building skills:
Helping participants develop confidence and independence in managing their supports.

Coordinating supports:
Making sure services work well together and align with the participant’s goals.

Problem-solving:
Assisting with challenges such as provider issues, plan changes, or unexpected situations.

Monitoring progress:
Checking that supports are meeting needs and adjusting if required.

Preparing for reviews:
Helping participants gather reports and evidence for NDIS plan reviews.

A Recovery Coach works alongside you to:
