Role Overview:
We are seeking a compassionate Chronic Disease Care Plan Nurse to join our wonderful team. In this role, you will play a vital part in coordinating and delivering chronic disease management services, including the development and implementation of General Practice Management Plans (GPMPs), Team Care Arrangements (TCAs), and follow-up reviews.

Key Responsibilities:
• Conduct comprehensive health assessments for patients with chronic and complex conditions
• Develop, implement, and review GPMPs and TCAs in collaboration with GPs and allied health professionals
• Educate and support patients in managing their chronic conditions, promoting self-management and health literacy
• Coordinate referrals to allied health providers and follow up on care outcomes
• Maintain accurate and up-to-date patient records and documentation
• Conduct telehealth appointments

Requirements:
• Registered or Enrolled Nurse with current AHPRA registration
• Experience desirable but not essential
• Excellent communication, organizational, and documentation skills
• Ability to work both independently and collaboratively
• Proficiency with clinical software (Best Practice) is an advantage
