Documents that are a questionnaire commonly used to measure a level of dependence are only used where the results add specificity to an already documented condition. You cannot use the diagnostic test in isolation.

Most admitted patients who are smokers or have recently ‘quit’ will do a questionnaire.

Look for documentation of ‘dependence’ or ‘dependency’.
