Role:
1. Develop and implement comprehensive support plans for NDIS participants, working closely with them to identify their needs and preferences
2. Coordinate and liaise with a range of service providers, including therapists, support workers, and community organisations, to deliver integrated care
3. Monitor and review participants' progress, adjusting support plans as needed to ensure they continue to meet individual needs
4. Maintain detailed case notes and records, ensuring compliance with NDIS and organisational policies and procedures
5. Provide guidance and support to participants and their families, helping them to navigate the NDIS system and understand their rights and entitlements
What we're looking for:
1. Minimum 2 years' experience in a similar NDIS support coordination or case management role
2. Strong understanding of the NDIS framework, eligibility criteria, and participant planning process
3.Excellent communication and interpersonal skills, with the ability to build rapport and collaborate effectively with a diverse range of stakeholders
4. Demonstrated problem-solving skills and the ability to think creatively to overcome challenges
5. Familiarity with relevant legislation, policies, and best practices in the disability and aged care sectors
6. Proficient in using case management software and technology to support service delivery
