Weight loss does not equal metabolic improvement? Surgery may rewire the body’s internal systems
New research from Duke University and Columbia University shows that gastric bypass surgery not only supports weight loss but also triggers profound metabolic changes that dieting alone cannot achieve.
In a study involving severely obese patients with type 2 diabetes:
Both the surgery and diet groups lost approximately 10 kilograms
However, only the surgery group showed a significant decrease in circulating branched-chain amino acids (BCAAs), which are strongly associated with insulin resistance
Why is this significant?
A reduction in BCAAs is linked to improved insulin sensitivity, better glucose control, and in many cases, remission of diabetes β€” often occurring before significant weight loss is observed.
Additional findings:
Gastric bypass surgery increases the secretion of GLP-1, a hormone produced in the gut that enhances insulin response
It also slows nutrient absorption, helping to stabilize blood sugar levels
This research underscores the underestimated role of the gut in energy regulation and glucose control. It suggests that future treatments for type 2 diabetes may be developed to replicate these surgical metabolic effects without requiring invasive procedures.

G’day everyone,
Over the past few decades, I’ve spent much of my career looking at the link between obesity and type 2 diabetes β€” and the real-life ways we can turn things around.
A while back, we ran a randomised controlled trial here in Melbourne comparing two approaches for people recently diagnosed with type 2 diabetes:
1️⃣ Traditional lifestyle and medication
2️⃣ Laparoscopic adjustable gastric banding (or β€œlap-band” surgery)
The results were nothing short of remarkable. After two years:
βœ”οΈ 73% of those who had the surgery went into full diabetes remission – meaning normal blood sugar levels without needing meds.
βœ”οΈ In the lifestyle group? Just 13% achieved remission.
βœ”οΈ On average, the surgical group lost over 20% of their body weight.
We weren’t surprised to see better blood glucose numbers, less insulin resistance, and fewer needing diabetes meds. What did surprise us was just how strongly weight loss itself predicted diabetes remission β€” not necessarily the method, but the amount of weight lost.
πŸ’‘ In fact, if someone dropped more than 10% of their body weight, their chances of remission skyrocketed β€” no matter their age, s*x, or starting weight.
🧠 So what’s the take-home message?
For people living with obesity and early-stage type 2 diabetes, meaningful weight loss β€” especially through surgery β€” may offer a real chance at remission, not just better control.
This isn’t about vanity or quick fixes. It's about evidence-based medicine and giving people back their quality of life.
We still need larger and longer-term studies β€” but these results were a powerful wake-up call: maybe it’s time we start thinking about weight loss surgery as a front-line therapy, not a last resort.

Night Eating Syndrome (NES) isn’t just about the occasional late-night snack β€” it’s a real, diagnosable condition that may contribute to obesity and binge eating.
In our study of over 400 participants, 11% met the criteria for NES. That means:
They weren’t hungry in the morning,
Ate more than half their daily calories after 7pm, and
Struggled with sleep at least 3 nights a week.
We found that NES was significantly linked with:
Higher body mass index (BMI),
Binge eating disorder (BED),
And, interestingly, being male.
But here’s the kicker: psychological distress (like depression and poor mental health) was only really a problem in those who woke up during the night to snack. These β€œnocturnal snackers” had:
Worse mental health-related quality of life,
Greater depressive symptoms,
And higher hunger levels.

In modern society, many individuals are faced with circumstances that contribute to weight gain, often outside of their control. Obesity is complex, and it’s far from just a matter of eating too much or not exercising enough. "Obesity is the result of many interacting factors, and it's not something that should be blamed on the individual."
The Dangers of Quick-Fix Weight Loss
One of the most damaging myths is the idea of β€œquick-fix” solutions. People often turn to extreme diets or rapid weight loss strategies, hoping to see instant results. However, I’ve seen time and time again that these methods are not only ineffective in the long term, but they also harm the body. Rapid weight loss leads to muscle loss, slows down metabolism, and can cause nutritional deficiencies. "These drastic measures may give you temporary results, but they can also lead to a dangerous cycle of weight gain and loss, which can harm your health in the long run."
The Importance of Scientific Treatment
What’s crucial, however, is that weight loss should be approached scientifically. GLP-1 therapies, such as Semaglutide, are evidence-backed treatments that help regulate metabolism, control appetite, and assist in sustained, healthy weight loss. I firmly believe that scientific treatments should be at the forefront of obesity management. "These treatments provide a much-needed solution for those struggling with obesity, allowing them to address the underlying metabolic issues and achieve sustainable weight loss without resorting to extreme diets."
