Do you or someone you support take more then 5 medications?
Did you know that you could be eligible for a DMMR review?
This is a review of the medications that you are taking.

A Domiciliary Medication Management Review (DMMR), also known as a Home Medicines Review (HMR), is an Australian government-subsidised service available to people with a disability living in the community who meet specific eligibility criteria.

The service is designed to help patients get the most from their medications, prevent medication-related problems, and reduce the risk of adverse events.

Eligibility Criteria
People with a disability often qualify for a DMMR because their condition results in complex medication regimens or chronic health issues. You may be eligible if you meet one or more of the following criteria:
You take five or more regular medications.
You have a chronic medical condition.
Your current therapeutic goals for your medication are not being met.

You have had significant changes to your medication regimen in the last three months.
You are experiencing symptoms that might be an adverse drug reaction.
You have difficulty managing your medications due to dexterity problems, impaired sight, or cognitive difficulties.
You attend multiple doctors or specialists.

How the DMMR Process Works
The DMMR involves a team approach between you, your GP, and an accredited pharmacist.
GP Assessment and Referral: Your general practitioner (GP) first assesses your needs. If they determine you meet the criteria, they will discuss the service with you, get your consent, and send a referral to an accredited pharmacist or a community pharmacy of your choice.

Pharmacist Home Visit: An accredited pharmacist will contact you to arrange a convenient time for a review in your home. During the visit, the pharmacist will review all your medications (including over-the-counter and supplements), how they are stored, and how you take them.

Over the next couple of days, our regions are forecasted to experience severe weather conditions. Extreme heat can lead to severe illness, hospital admissions and mortality. Prior to, during, and following periods of hot weather, maintaining a cool environment and adequate hydration is crucial.

Extreme heat or heatwaves are periods of unusually hot weather. Climate change is resulting in more hot days and more intense heatwaves in Australia.

WHO IS MOST AT RISK ?
Hot weather can affect everyone, but some people are more vulnerable:
*People aged 65 years and olde
*Babies and young children*
*Pregnant women
*People who are homeless
*People with some medical conditions including heart disease, lung disease, diabetes, kidney disease, neurological disease, and mental illness
*Socially isolated people
*Outdoor workers.

HOW TO COOL DOWN
Get out of the heat to a cooler area indoors or shaded area outdoors.
Loosen or remove clothing.
Start to cool down any way you can: use a cool-water spray, apply a cool, damp sponge or cloth, wet clothes and skin, have a cool shower or bath, apply ice packs or crushed ice in a damp towel on the neck, groin and armpits.
If you become unwell seek medical treatment

Inside the Home Safety:

Smoke Alarms: Install working smoke alarms on every level and test them regularly.

Escape Plan: Develop and practice a home escape plan with two ways out of every room, keeping keys accessible.

Kitchen Safety: Never leave cooking unattended; it's the leading cause of home fires.

Heating: Keep heaters at least 2 meters from combustible items like curtains, and never leave them or open fires unattended.

Matches & Lighters: Keep them out of children's reach and teach kids they are tools, not toys.

During a Fire Emergency
Get Out, Stay Out: If a fire starts, evacuate immediately and call Triple Zero (000) from a safe location; never go back inside.

Close Doors: Close internal doors as you escape to slow fire spread.

Stay Informed: Monitor local fire alerts via radio or official apps.

A Safety Visit assists people to reduce their risk of an accidental fire in the home and to safely escape

The heinous act of killing innocent people is not subject to opinion, but rather a blatant moral and intellectual failing, a warped form of expression that no sane person can justify or sympathize with.
No morally upright individual can support the killing of innocent civilians, irrespective of their religion, ethnicity, race, or gender.
RIP to the 16 individuals and condolences to the family friends and wide community of these people
