Are you a compassionate and dedicated individual with a social work degree (or similar qualification)? Do you have a minimum of 1 year of NDIS experience and a passion for making a positive impact in people's lives? If so, we want to hear from you!

Position: Support Coordinator
Location: Hybrid - Roxburgh Park Office and Work from Home
Days: 3 days a week
Rate: Competitive

Responsibilities:

Provide support coordination services to NDIS participants
Collaborate with clients, families, and service providers to develop and implement effective support plans
Conduct assessments and identify individual needs and goals
Assist in navigating the NDIS system and advocating for participant rights
Maintain accurate and up-to-date documentation
Qualifications:

Social work degree or similar qualification
Minimum 1 year of NDIS experience
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Excellent organizational and time-management skills
