🌿 Notes from the Field
Foundational Supports
Across Australia, conversations about Foundational Supports are growing as governments, communities and services work toward a more sustainable and inclusive support system.
From the field, one thing is clear:
People do not live their lives inside funding categories.
They live in communities.
They need connection, guidance, practical help to access services, and places where they feel safe to participate — whether that is employment, connection to nature, joining a community group, learning new skills, or simply having someone help navigate everyday life.
Foundational supports remind us that wellbeing is not only clinical.
It is social.
It is environmental.
It is relational.
Community-based supports — including employment support, wellbeing groups, nature programs, mentoring and practical life support — play a critical role in helping people stay connected and resilient.
When these supports are strong, specialist systems like the NDIS and Medicare can focus on the people who need them most.
At Your Hart 2 Hart, we see this every day through our community wellbeing programs, Green Pathways, Blue Pathways, and allied health services.
Supporting people to belong, participate and thrive is not an “extra”.
Strong foundational supports strengthen the entire care system.
It is the foundation.
