✨ What is an “internal bra” in breast surgery?

The term can sound confusing, but it does not involve placing an actual bra inside the body. Instead, it refers to a surgical technique that provides temporary internal support for implants.

🔹 How it works

* A dissolvable suture is placed to secure breast tissue to the chest wall/rib
cage.
* This acts like a “ledge,” helping hold the implant in place and enhancing
cleavage.
* Over about 3 months, the suture dissolves and the body’s natural capsule
forms to keep the implant supported.

🔹 When it’s used

* Particularly helpful in revision cases (remove/replace implants).
* When the old capsule needs to be removed.
* To maintain implant position until the new capsule develops.

Breast Reduction Surgery

🔹 A common misconception

One of the most frequent misunderstandings about breast surgery is that the ni**le is cut off and re-attached during a breast reduction or breast lift. In the vast majority of cases, this is not how the procedure is performed.

🔹 What actually happens

* The ni**le remains attached to the underlying breast tissue via a section
called the pedicle.
* Surrounding skin is carefully removed.
* The breast tissue, with the ni**le attached, is repositioned higher on the
chest.
* A new opening is created in the centre of the reshaped breast, and the ni**le
is sutured into this natural position.

🔹 Reduction vs. Lift

* Breast Reduction: Excess breast tissue and skin are removed, reducing size and
weight.
* Breast Lift (Mastopexy): Minimal or no breast tissue is removed – only excess
skin is taken away to reshape and lift the breast.
* This approach preserves the blood and nerve supply to the ni**le in most
procedures, which is an important consideration for both function and safety.

Breast Augmentation 340cc Round Implants

This patient was seeking upper pole fullness and an increase in the overall size of her breasts. Dr Cronin used Motiva Ergonomix 340cc moderate projection round implants to achieve this outcome.

The 'projection' of an implant refers to how far forward the implant pushes off the chest. In a round implant, the centre of the implant is the highest projected point. Compared to in a teardrop implant, the lower third of the implant is the highest projected point.

Photos are taken 6weeks after surgery.

Breast Augmentation- teardrop (anatomical) breast implants with fat grafting

This patient has tall height, full projection teardrop, 380cc breast implants in a custom medial dual plane position. Dr Cronin also performed autologous fat grafting and an internal bra fixation for medial cleavage.

In breast implant sizing, the 'cc' stands for cubic centimeters and refers to the volume of the implant.

A 'tall height' teardrop implant refers to the vertical dimension of the implant. It means the base of the implant is taller than it is wide and is designed to be longer vertically and narrower across the chest. They are excellent at creating upper pole projection and tall cleavage planes.

Photos are taken 6weeks after surgery. Over the next 3-6months, the breasts will continue to soften and settle in position.

08/09/2025

Often the discussion around round vs teardrop implants is simplified to 'Round gives more upper pole, teardrop looks more natural', but it's far more nuanced than this.

Both have their strengths and weaknesses. Round implants have a low rate of complications and can create an 'enhanced' look, but because it's base is round, in order to fit one your breast needs to be as wide as it is tall, and the ni**le needs to be right in the middle. They're a great option when appropriate, but don't fit every patient.

Teardrop implants can, for the same implant width, have different heights, different projections, and different points of maximal projection to line up with the ni**le. They can be customised to fit patients that may have breasts that are tall and narrow, or short and wider, and are often a better option for women that have, through breastfeeding or weight loss, a ni**le position that has descended down on the breast.

Selecting the correct implant for your own individual breast can be complex, but is pivotal in achieving your desired outcome. This is why during your consultation, Dr Cronin takes time to listen to your desired outcomes, analyse and measure your existing breast dimensions, and recommend and explain the implant that is best suited to you.

This patient had undergone significant weight loss of 55kg and suffered with symptomatic excess skin on the abdomen and flank (waist) areas and deflated, ptotic (sagging) breasts.

Dr Cronin performed a circumferential lipectomy procedure and bilateral breast augmentation with full projection teardrop (anatomical) breast implants of 390cc volume. This procedure took approximately 5 hours and the patient was an inpatient at Pindara Hospital for 6 nights.

Photos are taken at 6 weeks post-operatively, at which time the patient is able to resume all of her normal activities of daily living, including exercise. The breast will continue to soften and settle over the next 3-6months.
