The mons p***s is the rounded area over the p***c bone, often containing fatty tissue. In some individuals, this area becomes more prominent due to factors like weight gain, hormonal changes, or ageing. In certain cases, patients may seek surgical treatment to reduce or lift this area.

Abdominoplasty (tummy tuck) is a surgical procedure involving removal of excess abdominal skin and fat and, in some cases, tightening of abdominal muscles. This procedure is typically considered by individuals following weight fluctuations, pregnancy, or loss of skin elasticity.

In suitable cases, a reduction or lift of the mons p***s (monsplasty) can be performed during an abdominoplasty. This may improve the contour of the lower abdomen and p***c region.

During a breast reduction, surgeons often reposition the ni**le–areolar complex. This process relies heavily on maintaining an adequate blood supply through tiny vessels known as the dermal and subdermal plexuses.

Now here’s where va**ng becomes dangerous: ni****ne, whether from ci******es or v**es, is a potent vasoconstrictor. That means it causes blood vessels to narrow, reducing blood flow. Add to that the carbon monoxide exposure from smoke or v***r, which decreases oxygen delivery to tissues by binding to hemoglobin - the protein in your red blood cells that carries oxygen.

In surgery, your tissues are already stressed. If blood and oxygen delivery is compromised - which happens with ni****ne use - the tissue around the ni**le can become ischemic, and in some cases, die completely. This is called full-thickness necrosis, and yes, that means your ni**le can fall off.

Surgeons often require at least 6 weeks of no ni****ne - not just smoking - before and after surgery. Because losing your ni**le over a v**e? Not worth it.”**

When planning a breast reduction, we use the distance from the sternal notch (the dip at the base of your neck) to your ni**les to guide how much tissue can be safely removed. This helps protect the blood supply to the ni**les. If your ni**les sit well below that safe range and you’re seeking a very small size, we may need to consider a planned free ni**le graft—where the ni**le is repositioned like a skin graft, trading some sensation and breastfeeding ability for safety and a smaller breast size.

✅ If you’re staying on your GLP-1 medication before surgery (which is usually the case):
• 24 hours before surgery:
Switch to a clear fluid diet only — this helps clear your stomach safely without requiring you to stop the medication.
• ✅ Examples: water, clear broth, black coffee/tea (no milk), electrolyte drinks, pulp-free juice, jelly (transparent).
• ❌ No solids, dairy, thick soups, or meal replacements.
• 6 hours before anesthesia:
Begin full fasting (nil by mouth) — this includes clear fluids unless otherwise instructed by your anesthetist.

These steps are specifically designed to reduce the risk of gastric content retention and aspiration, which GLP-1 medications can increase due to delayed stomach emptying.

Breast reduction surgery is a common procedure aimed at relieving discomfort caused by overly large breasts. While generally safe, there’s a small but important risk to be aware of: in about 2.4% of cases, blood supply to the ni**le may be compromised during surgery. When this happens, surgeons may need to remove the ni**le entirely - a technique known as using a “free ni**le graft” - and then graft it back onto the breast. While this approach can preserve the appearance of the ni**le, it typically results in a loss of sensation and the inability to breastfeed.
