At EES we use the ZEISS Cirrus 6000 with Angioplex OCT Angiography, a cutting-edge eye scanner that gives our specialists the clearest, most detailed view of your retina (the back of your eye).

Two scans in one:
🌟 OCT: Takes cross-section “slices” of your retina (like an MRI for your eye)
🌟 OCTA: Maps and examines tiny blood vessels without dye injections (unlike traditional angiograms)

A 41-year-old male presented with central serous chorioretinopathy (CSCR) - but without the usual risk factors like stress, steroid use, or a Type A personality. On deeper questioning, he revealed recent use of topical minoxidil for hair loss.

🔬 Possible Mechanism
Minoxidil may increase VEGF expression and cause choroidal vasodilation, potentially raising vascular permeability, leading to subretinal fluid accumulation seen in CSCR.

📌 Case Takeaways
▫️ CSCR from minoxidil is rare, with only a few case reports
▫️ Risk may increase with prolonged use or higher concentrations
▫️ Always ask about all medications and supplements, even if not initially volunteered and don’t hesitate to search uncommon associations

2. Observe — most cases resolve spontaneously within 3–4 months

Dry eye disease is more than just occasional discomfort — it’s a chronic condition that can affect your daily life.

🔎 Here are common symptoms to watch for:
✔ Burning or stinging sensation
✔ Redness and irritation
✔ Blurry vision (that improves with blinking)
✔ Watery eyes (yes, ironically!)
✔ Gritty feeling, like sand in your eyes
✔ Light sensitivity
✔ Difficulty wearing contact lenses
✔ Eye fatigue after screen time

⚠️ Risk factors: Aging, screen use, dry climates, certain medications, and autoimmune conditions (like Sjögren’s syndrome).

👁️ VMT – Watch or Operate?
Vitreomacular traction (VMT) can go in different directions:
▫️ Sometimes it resolves spontaneously – no surgery needed (10-30%)
▫️ Sometimes it progresses – and causes a full-thickness macular hole needs surgery
▫️ Persistent VMT that does not resolve on its own and causes causes visual symptoms such as central blur or distortion – this also would generally need surgery

In this case the VMT spontaneously resolved!

Many patients are referred to a retinal specialist for monitoring and management, but not always surgery right away.

📌 Case Study: Normal Tension Glaucoma in a Myopic Eye

🔍 Findings:
▫️ Fundus image shows a myopic disc with thin inferior rims
▫️ Red-free imaging reveals an RNFL wedge defect OU
▫️There myelinated nerve fibers in the left eye inferiorly
▫️ Structure-function correlation is evident with RNFL loss, ganglion cell loss on macular OCT and a corresponding defect on visual fields

💊 Treatment:
Started with Xalatan initially, then escalated to Ganfort PF — aiming for lower long-term IOP as it was a younger patient
▫️ NTG can be trickier to manage — harder to drop IOP significantly when baseline is already “normal”
▫️ In NTG, it’s not just about hitting a single IOP measurement in clinic — may need to look into reducing diurnal fluctuations

➡️ Next steps for treatment:
If he progresses further, surgery could be considered — but only after weighing risks vs benefits
▫️ Patient is young, newly diagnosed, and trust is required to develop in the treatment journey
▫️ When progression occurs despite combination drops, the next step would be SLT laser then surgery

A young gardener presented with a sore eye after being struck by a tree branch. On exam: corneal abrasion—but the key lesson? Always look deeper.

🔍 Findings:
▫️ Subconjunctival foreign body (FB) embedded beneath the conjunctiva
▫️ Corneal abrasion

✋ Why Refer?
This FB required microsurgical removal

💊 Treatment:
1. Small incision in the conjunctiva
2. Extraction with micro forceps
3. Saline washout to eliminate residual debris
4. Prescribed chloramphenicol qid
5. Follow up to ensure healing and no signs of fungal infection
