Cramps, nausea, headaches… periods can be uncomfortable, to say the least. A big part of this is due to hormone-like compounds called prostaglandins, which trigger uterine contractions to help shed the lining during your cycle.

In endometriosis, prostaglandins also affect lesions – the tissue similar to the uterine lining growing outside the uterus – which causes additional inflammation and pain. This is why anti inflammatory drugs, like ibuprofen, are commonly used to relieve its effects: they work by reducing prostaglandin production, easing cramps and other symptoms.

When it comes to endometriosis surgery, understanding the difference between excision and ablation matters.

Excision involves cutting out the lesion along with a small portion of surrounding tissue. These samples can then be sent for biopsy confirmation.

Ablation refers to the destruction of the lesion's surface using heat (e.g. diathermy or laser), without removing the tissue entirely.

At Sydney Endometriosis, excision is our preferred method. It’s considered the gold standard for treating deep infiltrating endometriosis, with lower recurrence rates, less risk of adhesions, and better outcomes for pain relief. By contrast, ablation is a more superficial approach and may carry a higher risk of scarring.

SURGEON’S EYE VIEW | Deep Endometriosis of the Re**um

Ultrasound imaging is not only essential for diagnosing endometriosis, it also plays a crucial role in guiding treatment and surgical planning. In this case of deep endometriosis involving the re**al wall, an irregularity seen on ultrasound (Image 2) indicated the presence of a lesion, which was later confirmed during surgery by a colore**al surgeon (Image 3).

The latest report from the Australian Institute of Health and Welfare highlights the measurable impact of endometriosis on those living with the condition.

The Australian Burden Of Disease Study 2024 has found endometriosis represents 0.5% of the non-fatal burden among females in Australia. This burden is measured in Years Lived in Disability (or YLD), which reflects the years of healthy life lost due to living with illness or injury. The rate for endometriosis has increased from 0.5 years per 1,000 females in 2003 to 0.7 in 2024, after adjusting for differences in age which can be due to changes in its prevalence, awareness or diagnosis.

Dr Mike Armour, consulting Traditional Chinese Medicine practitioner at Sydney Endometriosis, has co-authored a study with the University of Western Sydney to explore the drivers and barriers of cannabis use for endometriosis symptom management.

The study, which spans 28 countries, reveals that cannabis is viewed as ‘superior to pharmaceuticals’ in both effectiveness and side-effect profile overall, however there are significant obstacles – and not only legal restrictions. This includes stigma around drug use, and fears around social and workplace judgment, leading some to not disclose their use to doctors.

Bindi Irwin sheds light on a reality many living with endometriosis face, in a new campaign by the World Health Organisation to raise awareness and push for greater action. After years of pain, emotional distress and dismissal while seeking answers, it wasn’t until surgery that doctors discovered 37 lesions and a cyst.

She has been open about her journey and a strong advocate for the endo community since. “Emotional support is just as important as medical care,” she says, highlighting the vital role of empathy alongside clinical treatment for a condition that impacts one in 10 women and people AFAB worldwide.

Her voice adds to a growing call for change – one that urges both compassion and action in endometriosis care.

Australia is taking a step forward in endometriosis care with the launch of the Endometriosis Management Plan (EMP) – a new online clinical support tool developed by RACGP and Monash University. Now being trialled in eight medical centres nationwide, the EMP aims to equip GPs with the tools and training to recognise symptoms early, set goals and provide evidence-based treatment plans.

The goal is to enable earlier intervention, improve outcomes and ease the personal and economic burden of this chronic condition.

Ultrasound is a powerful, non-invasive imaging tool in the diagnosis of endometriosis – helping to identify lesions and inform surgical planning.

In this scan, key anatomical structures of the female pelvis are clearly visible: the va**na, bowel, rectova**nal septum (the tissue separating the re**um and va**na) and the Pouch of Douglas – the space between the uterus and re**um, where deep endometriosis can often be found.
