𝗥𝗶𝗯 𝗙𝗹𝗮𝗿𝗲𝘀 𝗮𝗻𝗱 𝗗𝗶𝗮𝘀𝘁𝗮𝘀𝗶𝘀⁣

I’ve seen too many people tell me their concerns about if their ribs are flaring or not. It would seem that social media is causing unnecessary and likely unintentional harm by doing this!⁣

I first came across these ideas via Diane Lee and LJ Lee but Greg Chaplin from has a nice blog post which has references back to 1918 about the ISA - btw, I don’t necessarily agree with Diane, LJ, or Greg about ISA, just giving credit where it is due!⁣

“Normal Infrasternal Angle (ISA)” is touted as 90deg. Less and it is the fault of external obliques being overactive. More and it is the fault of internal obliques being overactive. But when and how do you measure it? If I measure, it is usually during “quiet breathing” and it should move.⁣

More and more seems to be made about the ISA, especially with diastasis.⁣

What many people forget is that upper TrAb will narrow the ISA if the ribs are moving. But if the ribs are fixed (braced), then it will pull on the Linea Alba.⁣

There simply isn’t enough research to assert FOR CERTAIN just why people have specific shapes. It’s like athleticism / “beauty” - it’s hard to describe but you know it when you see it!⁣

We can’t go around generating fear of trying to put people into boxes or “archetypes”. Just meet the person where they are at!⁣

Also, if you think you’re cueing TrAb by feeling at the medial edge of the ASIS and it “pops up”, I’ve got news for you (again!) - it is NOT TrAb! It’s probably internal obliques being contracted.⁣

What if the “rib flare” you are keen to diminish is because you are teaching TrAb incorrectly?!⁣

And no, your results don’t prove anything about the theory - post hoc fallacy is a real thing!⁣

I teach this sort of stuff in my course Diastasis Done Differently. You’ll see the anatomy and ultrasound images about the different muscles.⁣

Why not do the course online or come in person?⁣

Hot take: Return to exercise after an Abdominoplasty is not that different to return to exercise post C-Section or abdominal surgery.⁣

Yes, it is a significant operation and not to be taken lightly. It should be prepared for and the person needs support for at least 4 weeks including no picking up kids and weights / things.⁣

It’s why I recommend to wait until the person’s youngest is at least 3yo but you *might* have a chance of explaining why they can’t be picked up.⁣

There’s a lot of depth of knowledge & variables if you want to get into the minutae - but I don’t think it is necessary for return to exercise!⁣

I advocate for working by .⁣

Principle 1 - always recommend the person follows the surgeon’s recommendations - no matter how silly they might be, if something goes wrong, you don’t want to be the one held responsible. Advocate for your client with the surgeon etc but at the end of the day, I just go with what the surgeon says.⁣

Principles 2 - Healing takes time. It is easier to take something a bit tight and make it a bit looser. It is HARD to take something too loose and make it tighter. Trust the process, don’t rush it. Slow is smooth, smooth is fast!⁣

Principle 3 - Load within the tissue limits. Consider the effect of fatigue with load & remember that just being upright is load on the tissues. It’s why binders are used. Sometimes they aren’t but I have no issue with them if they aren’t causing perineal descent…which leads to…⁣

Principle 4 - mind the PF and Perineum. A more distensible (mobile, less “tight”) abdomen can offset IAP. Abdominoplasty will change that. Ensure the PF is working well to support the pelvic organs BEFORE the operation.⁣

Principle 5 - don’t worry so much about TrAb, breathing, & all the core things. 3D segmental motion is probably more helpful (including breathing !⁣

Principle 6 - The rest of the body probably needs more care from the recovery - back, neck, hips etc. Remember them!⁣

That’s it! Sounds like post C-Section or Abdo surgery RTE guidelines huh? 🤔⁣

Have you ever thought “that doesn’t make sense” to things you hear or see or are taught? 🤨

Several years ago I (Antony), had a good hard think about myself as a therapist. 🧠

I have always helped a lot of my clients get better, however I was confused when people got better despite the fact I did a technique incorrectly, or they didn’t ‘do their homework’. 🤔

But what was even more confusing was when I knew I did everything perfectly by the textbook, and they didn’t get better like they should. 😵‍💫

It didn’t sit right with me, and I went on a 3 year journey to change the way I worked and thought about the way I think, learn, assess, and provide management for the people who come to see me. 🙋🏽‍♂
