🌿 Important Update: Support at Home Reforms 🌿

From 1 November 2025, the Australian Government’s new Support at Home (SAH) Program officially replaces the Home Care Packages (HCP) and Short-Term Restorative Care programs.

This reform aims to make aged care simpler, fairer, and more flexible — helping older Australians stay independent and supported in their own homes for longer.

Under the new Support at Home program, services will include:

💙 Nursing and clinical care
💙 Personal care and daily living assistance
💙 Allied health and therapy services
💙 Home maintenance, cleaning, and meal support
💙 Transport, social and community participation
💙 Assistive technology & modifications

At Heart to Hand Healthcare Services, our nurse-led team is here to support clients through this transition with compassion, understanding, and continuity of care.
We are committed to ensuring that every client continues to receive the quality services they deserve — seamlessly under the new Support at Home framework.
