We are excited the announce that the EMD x NOMAD Rural Emergencies Camp is back for 2025, brought to you by our proud partners; Deakin’s Western Regional Training Hub and VRGP.

This year we have some amazing clinicians joining us to provide their wisdom, skills and years of experience running stations to develop your skills. The SES are also returning to run a hands on simulation event. Stay tuned for further details on what we have in store!

What types of injuries cause compartment syndrome?

Fractures: account for about 75% of compartment syndrome. Most commonly the tibia, humeral shaft, combined radius.

Soft tissue injuries such as:
crush injury
snake bite
excessive exertion
prolonged immobilisation
constrictive dressings and plaster casts
soft tissue infection
seizures
extravasation of intravenous fluids and medications
burns
tourniquets

What is the pathophysiology of compartment syndrome?

Muscles are contained within inelastic fascial sheaths. When injured muscles swell, a hematoma forms, or external compression is applied, the pressure within the fascial compartment increases.
If the pressure rises high enough, capillary pressure is exceeded and circulation is compromised. This results in ischemic injury to the muscles and nerves.

How quickly does irreversible injury occur with compartment syndrome?

Ischemic injury to muscles and nerves occurs after 4 hours of complete ischemia.
This becomes irreversible at some point over the next 4 hours (i.e. 4-8 hours after the onset of ischemia), resulting in local rhabdomyolysis and neuropraxis progresses to axonotmesis as nerve injury worsens.

A 24 year old woman fell on an outstretched hand while playing netball
• Describe the findings on the X-ray
• What complications are associated with this injury ?
• What is the preferred management ?

This X-ray shows a classic case of a “Galeazzi” fracture dislocation. This is a distal 1/3 radial shaft fracture with an associated distal radioulnar joint (DRUJ) injury, along with palmar dislocation of the distal radius. Clinically their presentation with pain and a deformed distal forearm is indistinguishable from that of the more typical fractures around the wrist. Galeazzi fractures are associated with a number of long term complications, especially when unrecognised or inadequately treated. They can cause long term instability of the DRUJ, limited forearm ROM, chronic wrist pain and osteoarthritis. In adults these fractures nearly always require open reduction and internal fixation. The radial fracture will usually be plated and then the DRUJ stabilised either with a plaster or percutaneous pin fixation. In children, Galeazzi fractures seem to be less often associated with long term DRUJ instability and may be treated with a closed reduction and long arm cast but careful orthopaedic follow up is still required.
