Do you have a good relationship with your pharmacist?

If you are starting to struggle with dosages and taking medications at the correct time then it might be worth chatting to your pharmacist to see if they can organise a webster pack to help you.

A lot of pharmacists will also be able to administer the flu vax for you.

Or maybe you need more regular Blood Pressure Monitoring, ask your pharmacist if this is something they can help you with.

And always ask if it is possible to arrange the delivery of your medication.

If you are looking for a mobility or safety aid here are some questions to ask.

What service is offered by the provider?

Is there any warranty?

Where do I send my mobility scooter if repairs are needed?

Do you offer a replacement scooter while mine is being repaired?

How far away from the base will the alarm work?

Who is notified when my alarm goes off?

Is it an Australian company?

How long is the battery or charging life?

Who will install my aid?

And last but not least remember to ask lots of questions, your safety is number 1. Do your research.

This is a beautiful story of care and dignity. Well done to all involved.

Late last year the ERH community palliative care team received a referral from Rochester and Elmore District Health Service hostel for a 93 year old gentleman by the name of Jack.
Libby, one of our palliative care nurse consultants went down to Rochester to meet with Jack and provide advice on any symptoms he may have had. What Libby discovered was that Jack had a dying wish to visit his beloved family farm in Romsey.
Jack had lived on this farm for 91 years before moving into aged care two years ago. As Jack was very frail, he was unable to travel by car and needed an ambulance transfer.
Libby made many many phone calls determined to make this happen for Jack. The Royal Flying Doctor Service of Australia Echuca Service offered to do this free of charge under compassionate grounds.
Two days later, Jack spent the day at his farm with his family. By all reports from his family, Jack had the most wonderful day.
Jack passed peacefully away a few days later.
The family were so grateful for all of Libby’s hard work and to her surprise, some beautiful flowers were delivered to her at work the day after Jack died.
More than a few tears were shed by our team and this shows the power of health services and providers working together with patient centered care at the forefront of all that we do. Rest in peace Jack.
