Bilateral Ni**le Sparing Mastectomy with Bilateral DIEP Reconstruction

6 months post Stage 2 (Abdominal scar revision and bilateral reconstruction revision with removal skin paddles)

Breast reconstruction using an abdominal flap is usually a multi-stage process.
The initial surgery transfers tissue from the abdomen to create the breast mound.
Stage 2 is the refinement stage, where Dr Ahn focuses on shaping the breasts, improving symmetry and revising scars.
There are several unique options that Dr Ahn may use in this 2nd stage:
• Breast shaping and symmetry - Adjusting contour and projection for a more natural look and better balance between the breasts
• Scar revision - Improving the appearance of scars from the first surgery, including abdominal and breast scars
• Fat grafting - Small amount of fat are added to the breasts to smooth irregularities and improve fullness
• Breast reduction - liposuction or lift and reduction techniques can be used where one breast is larger than the other.

This stage is highly customisable and Dr Ahn works with the patient to address their goals in line with their anatomy.

Bilateral Upper Eyelid Ptosis Correction

This patient was born with severe bilateral congenital upper eyelid ptosis, making it difficult to open her eyes for over 30 years. She routinely had to tilt her head back to see and used her brow muscles to assist eye opening.

Congenital ptosis can be challenging to address. In infancy, accurate assessment of the levator muscle (responsible for eyelid elevation) is limited. If the problem is not severe and does not cause functional problems (eg visual issues) surgical correction is delayed until the child is older.

For adults that have some degree of levator function, levator shortening procedures can provide a significant improvement in eyelid opening. Compared to other methods, the effectiveness can be greater and avoid issues such as significant lagopthalmos (inability to close the eye) which can happen with frontalis suspension techniques (the common method used to address severe ptosis in infants).

1 in 8 women will be diagnosed with breast cancer in her lifetime. It remains the most common cancer affecting women and the leading cause of cancer-related deaths in Australia.

Early detection of breast cancer significantly improves survival rates. Studies show that the five-year survival rate for patients diagnosed with Stage I breast cancer is over 98%

It is recommended women are aware how their breasts normally look and feel - any changes should be reported to their GP quickly. Look out for new or changed lumps, nipole discharge, changes in skin colour, skin texture or experiencing pain in the breast.

Bilateral Upper Eyelid Ptosis Correction

Ptosis is when the upper eyelid droops, which can make eyes look heavy, tired or uneven - and in some cases can block vision.

Ptosis correction surgery lifts the eyelids to restore a more open, refreshed appearance and improve visual function.

In some cases, this procedure may be eligible for a Medicare rebate. Dr Ahn performs this procedure under local anaesthetic when completed in his Chatswood Rooms or with sedation when in hospital.

Bilateral Incisional Double Eyelid Blepharoplast with Bilateral Medial Epicanthoplasty

In some patients, a medial epicanthoplasty is an important step to include in double eyelid procedures to achieve a balanced and natural eyelid result.

Whilst incisional and non-incisional blepharoplasty techniques create the eyelid crease, the inner corner fold (epicanthal fold) can sometimes limit how well the crease is defined, or create tightness at the inner corner that disrupts the smooth contour of the eyelid.

Bilateral Upper Eyelid Incisional Double Eyelid Blepharoplasty. 2 part surgery.

This patient underwent double eyelid surgery in two stages - an approach that is sometimes needed to achieve the most balanced and refined results.

Eyelids are one of the most complex anatomical areas on the face, with subtle asymmetries and differences in skin, fat, and muscle tone between sides. Small variations in tissue response, healing, or underlying asymmetry may become apparent once postoperative swelling has fully resolved. In these cases a second-stage procedure can be considered to address residual imbalance or refine crease definition.

This approach isn't planned for every patient from the outset - but when needed it allows for targeted adjustment and a more balanced and subtle result.

Dr Chris Ahn recently presented at Northern Beaches Hospital’s Grand Rounds, speaking with colleagues about breast reconstruction options following mastectomy.

In Australia, 40% of patients diagnosed with breast cancer undergo a mastectomy, yet only 1 in 5 of those patients go on to have a reconstruction. In contrast, overseas that number is closer to 50%.

The difference isn't surgical ability - its patient education and timely referral. Too many patients are either unaware that reconstruction is an option or receive the information too late to consider it as part of their treatment plan.
While reconstruction may not be appropriate for every individual, all patients deserve the opportunity to make an informed decision early in their breast cancer journey.

Engaging hospital teams, including medical, nursing and allied health staff, is key to improving access and outcomes in breast reconstruction.
