MEET 1 OF OUR MAJOR SPONSORS

Home Caring Vermont – Quality Care with a Personal Touch

At Home Caring Vermont, we pride ourselves on delivering compassionate, tailored care across the NDIS, aged care, nursing, and private service sectors. As a registered NDIS and Aged Care provider, we offer a full range of supports designed to meet participants’ unique needs and goals. Our services are guided by nursing-led case management, ensuring that health and wellbeing remain at the centre of every care plan.
Our qualified nurses bring current skills and experience from the hospital system, meaning participants benefit from clinical knowledge that is up to date with best practice. Our nursing services include pressure care, wound dressing, catheter care, and medication administration, along with ongoing monitoring and support. This combination of hospital-based expertise and community-focused care ensures our participants receive safe, professional, and high-quality support at home.
We support participants with transitional care from hospital to community. For NDIS participants requiring Medium Term Accommodation (MTA) while seeking a permanent suitable home, we partner with trusted accommodation providers to ensure a smooth transition. In addition, we can provide Supported Independent Living (SIL) arrangements, offering both housing and care supports tailored to each participant’s individual goals for independence and quality of life.

We also provide services to Home Care Package (HCP) participants as well as privately funded clients who are seeking personalised support. Whether it’s aged care, nursing supports, or assistance with daily activities, our services are tailored to meet individual needs with flexibility and respect.
Home Caring Vermont is led by an experienced team with both professional and lived expertise.
Lara Furletti – Managing Director of Home Caring Vermont is a Registered Nurse with over 20 years of experience across aged care and hospital. Lara also spent more than 10 years with the Department of Human Services as a Disability Support Officer, working in community residential units supporting people with high support and complex medical needs. As both a health professional and a parent of a son with a disability, Lara brings invaluable insight, compassion, and advocacy to her role. She has
also completed qualifications in Support Coordination, Community Sector Management, and Disability, further strengthening her ability to lead services that are person-centred and clinically informed.
Shannyn, a Senior NDIS Case Manager and Psychosocial Recovery Coach who also serves as Support Coordinator for NDIS participants. With over 25 years experience in both government and non-government sectors—ranging from frontline support work to senior management—Shannyn offers deep understanding and strong leadership. As a mother of multiple children with complex disabilities, she also walks the NDIS journey personally, bringing empathy, determination, and first-hand knowledge to her advocacy and case management. Her qualifications include a degree in Disability and Psychology, Business Management in Health and Community Services, and certification as a Trauma-Informed Counsellor, among many other industry-specific credentials. This wealth of knowledge, combined with her lived experience, ensures participants and their families receive the highest level of care, advocacy, and support. Importantly, she also provides direct care herself when needed—maintaining a personal touch that reflects our commitment to quality services.
Together, Lara and Shannyn combine professional expertise with lived experience to ensure Home Caring Vermont delivers high-quality, compassionate, and effective supports for every participant.
At Home Caring Vermont, our values are simple: listen, support, empower, and deliver care that creates meaningful outcomes. Whether it’s NDIS, aged care, Home Care Packages, nursing, transitional care, or private services, we’re here to help people live the life they choose with dignity and independence.
