Why do we always ask for your personal details?
On your arrival - we will start with giving you a big smile and a cheery hello!
You will be asked to confirm your personal details by a member of our friendly reception team, at EACH visit. Yes, each visit, as things can change, and we endeavour to keep your file as up to date as possible at all times.
PLEASE be patient and respectful with receptionist staff as this a vital part of their role in keeping YOUR record up to date.
The following questions are required every time you visit us, but we will endeavour to work though them as quickly and efficiently as possible:
We will ask you -
- Your name
- Mobile number
- Your home address
- Your email address
- Who is your usual Doctor
- Your Medicare Card
- Next of Kin and Emergency contact
- If you identify as Aboriginal or Torres Strait Islander
- If you are on concession card (Pension card or Health Care Card)
- If you have any cough, runny nose, sore throat, fever or cold symptoms. If you do, reception staff will instruct you to pop on a mask and may ask some further questions or request you perform a RAT test before seeing your Doctor. Please respect our staff. Masks are important to protect other patients in the practice and staff.
