✨5 Documents You Actually Need for Your NDIS Application✨

Before you begin your NDIS application, you'll need some documents to prove your identity, residency, and disability.

✅Your proof of age document can be your driver's license, your birth certificate, or a passport.
✅For the proof of residency, provide your Australian birth certificate, a citizenship certificate or a valid visa.
✅Now, you must provide evidence of your disability with medical reports and/or assessments from registered health professionals. These assessments must outline how you are impacted in areas such as daily life, social interactions, and ability to work or study.
✅Next up is filling out your Access Request Form (ARF): This is the main form you'll need to complete to apply for the NDIS.
✅Another form you may require is a Supporting Evidence Form (SEF): This form can be used to provide further evidence to support your application if needed.

If you are unsure about any part of the application process, contact the NDIS directly or seek assistance from a Local Area Coordinator (LAC).
