GM EXCELSIOR students I notice that many are visiting my site that FB would not take down. This is from 2014 . I am no longer following the SG and many things had changed. So thread cautiously. Use what I can and follow EC guidelines TY.

Gd afternoon, when you get your assignment, choose your nanda by the abc method. Airway, breathing and circulation. look at what u are assigned, your interventions are there. Ask the RN the pertinent questions: What was the last pain level, when last medicated, does the pt take their pills whole or crushed. How is their gait, last vs (if she does not know the ce will get it for you) If your pt was medicated within the last 4 hrs., and it is not time to medicate again, then declare cdM, TELL the RN, return to pt. and do 2 more things, the 3rd being that pt. will be medicated, when the RN calls md and get a BREAKTHROUGH pain med.

If a pt refuses an intervention offer twice, document accordingly, do not offer more times, do not change care plan, no need to, and complete your pcs.
