Diagnosing myelopathy, especially early, is not straightforward.

It requires medical practitioners being aware of DCM and then pulling three (3) key sources of information together.

These are:

โ€ข a physical examination and
โ€ข MRI (Magnetic Resonance Imaging) of the cervical spine

Wear & tear as we age along with its corresponding symptoms cause many doctors to misdiagnosis patients leaving DCM to progress, worsening over time.

Most people wait years for a diagnosis as this condition is still very misunderstood leading to a poorer prognosis once discovered.

Cervical myelopathy can affect the brain as was confirmed in a study done in 2020.

The messages through your spinal cord go both ways โ€” from your brain to your body & vice versa. When cervical myelopathy starts to progress, it limits the ability of your brain to interpret what your body is saying.

This can feel like numbness & tingling, cognition issues (โ€œbrain fogโ€œ), and a lack of spatial awareness.

Cervical myelopathy is a progressive condition. This means that symptoms grow worse over time.

After the initial stage, which consists mostly of neck pain and stiffness, other, more severe symptoms may begin to appear.

These ๐ฌ๐ฒ๐ฆ๐ฉ๐ญ๐จ๐ฆ๐ฌ ๐ฆ๐š๐ฒ ๐ข๐ง๐œ๐ฅ๐ฎ๐๐ž:

โ€ข confusion or cognitive difficulty

โ€ข tingling or numbness in your extremities

Many of these symptoms are disregarded as common side effects of ageing.

No, we are not referring to the euphemism for an annoying person.

Along with the symptom noted above, some of the early signs of Cervical Myelopathy might include:

๐Ÿ”น neck stiffness
๐Ÿ”น limited range of motion
๐Ÿ”น fine motor clumsiness, such as difficulty holding a pen or counting out change
๐Ÿ”น headache that stems from nerves in your neck (occipital neuralgia)

As a person ages, the disks in their spine start to lose height & begin to bulge. They also lose water content, dry out & become stiff. The result is collapse of disk space & loss of space height.

When this happens, the vertebrae move closer together & the body responds to a collapsed disk by forming bone spurs in an attempt to strengthen the disk.

Bone spurs cause stiffness & narrowing of the spinal canal and start to compress the spinal cord.

15/07/2022

It is caused by any condition that puts pressure on the spinal cord and can occur anywhere in the spine, including in the neck (cervical spine) & the torso (thoracic spine).

Symptoms of spinal cord compression may develop suddenly or gradually.

Potential causes of spinal cord compression include:
โ€ข Degenerative conditions
โ€ข Rheumatoid arthritis
โ€ข Disk Herniation
โ€ข Injuries
โ€ข Bone Spurs
โ€ข Tumors

Christine Cocanour, MD, FACS, professor of clinical surgery at UC Davis Health in Sacramento, California, and chair of the ACS Committee on Trauma's Best Practices Guidelines Committee, told Medscape Medical News:

"As our committee thought about potential topics for a new best practices guideline, spine injury was a leading topic, because although it is relatively rare, it can have devastating consequences, and there are a number of management areas that remain controversial."

In nonoperative injuries, the new ACS guidelines call for discontinuing bracing when possible in favor of early patient mobilization.

If surgical stabilization is needed, it should occur early in the treatment algorithm.

The gut reaction of many nonโ€“spinal trauma specialists is to immobilize the area involved rather than risk permanent spinal cord injury.

But this may often be unnecessary & may prevent much-needed early mobilization.

Simply put, spinal cord injury is any injury that causes damage to the spinal cord or nerves at the end of the spinal canal.

Therefore, persons with arthritis changes in the bones of the neck are predisposed to CCS if any forceful extension of the neck occurs e.g. car accident.

In such situations, the canal through which the spinal cord travels can be narrow & the spinal cord can be squeezed. There is usually no obvious break or fracture in the bones of the neck and spine may be stable.

When the spinal cord is squashed, bruising, bleeding & swelling can occur, particularly in the center or central portion of the spinal cord.

Since the spinal cord is organized with the nerves that control the movement of the arms in the center & the nerves going to the legs more toward the outside, the arms are affected more than the legs in this situation.

The injury is considered โ€œincompleteโ€ because patients are usually not completely paralyzed.

It results in weakness in the arms more so than the legs.

E.g., if a stroke causes an area of brain damage, other parts of the nervous system can take over the lost function of the damaged area.

Traumatic spinal cord injuries (SCI) have devastating lifelong sequelae for affected individuals, caregivers, & society. Modest improvements in sensorimotor function can significantly enhance quality of life and decrease costs of care.

Why is early surgical intervention critical?

โžก๏ธEarly surgical decompression of the injured spinal cord is one of the few available interventions that can potentially alter the long-term recovery trajectory for this devastating condition.

Early decompression has a sound pathophysiologic rationale, clinical evidence of efficacy, & international guideline recommendations as a treatment option.

Anna Gonzales had numbness & falls for months. She was offered surgery, but doctors were cautiously optimistic as there was a 20% to 30% chance of success.

Anna then had a minor fall in a parking lot and was paralyzed from the neck down. Doctors noted she had a badly damaged spine.

You know that feeling when you sip an ice cold slushy or milkshake ๐Ÿฅถ๐Ÿฅถ๐Ÿฅถ

This pain occurs when cold hits the receptors in the outer covering of the brain, called the meninges. The cold creates a dilation & contraction of arteries, causing a rapid-onset headache.

Synopsis:

Leonard (Guy Pearce) is tracking down the man who killed his wife. The difficulty, however, of locating his wife's killer is compounded by the fact that he suffers from a rare, untreatable form of memory loss. Although he can recall details of life before his accident, Leonard cannot remember what happened 15 minutes ago, where he's going, or why.
