Diagnosing myelopathy, especially early, is not straightforward.

It requires medical practitioners being aware of DCM and then pulling three (3) key sources of information together.

These are:
• a person’s symptoms
• a physical examination and
• MRI (Magnetic Resonance Imaging) of the cervical spine

Cervical myelopathy can affect the brain as was confirmed in a study done in 2020.

The messages through your spinal cord go both ways — from your brain to your body & vice versa. When cervical myelopathy starts to progress, it limits the ability of your brain to interpret what your body is saying.

This can feel like numbness & tingling, cognition issues (“brain fog“), and a lack of spatial awareness.

Cervical myelopathy is a progressive condition. This means that symptoms grow worse over time.

After the initial stage, which consists mostly of neck pain and stiffness, other, more severe symptoms may begin to appear.

These 𝐬𝐲𝐦𝐩𝐭𝐨𝐦𝐬 𝐦𝐚𝐲 𝐢𝐧𝐜𝐥𝐮𝐝𝐞:

• difficulty walking or keeping your balance
• confusion or cognitive difficulty
• loss of bladder control
• tingling or numbness in your extremities

Many of these symptoms are disregarded as common side effects of ageing.

No, we are not referring to the euphemism for an annoying person.

Along with the symptom noted above, some of the early signs of Cervical Myelopathy might include:

🔹 neck stiffness
🔹 limited range of motion
🔹 fine motor clumsiness, such as difficulty holding a pen or counting out change
🔹 headache that stems from nerves in your neck (occipital neuralgia)

As a person ages, the disks in their spine start to lose height & begin to bulge. They also lose water content, dry out & become stiff. The result is collapse of disk space & loss of space height.

When this happens, the vertebrae move closer together & the body responds to a collapsed disk by forming bone spurs in an attempt to strengthen the disk.

Bone spurs cause stiffness & narrowing of the spinal canal and start to compress the spinal cord.

𝟐𝟑 𝐌𝐚𝐫𝐜𝐡 𝟐𝟎𝟐𝟐 - New recommendations were recently released by the American College of Surgeons' (ACS') Trauma Quality Programs regarding 𝘂𝗽𝗱𝗮𝘁𝗲𝗱 𝗴𝘂𝗶𝗱𝗲𝗹𝗶𝗻𝗲𝘀 𝗳𝗼𝗿 𝘁𝗵𝗲 𝗲𝗮𝗿𝗹𝘆 𝘁𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁 𝗼𝗳 𝘀𝗽𝗶𝗻𝗮𝗹 𝗶𝗻𝗷𝘂𝗿𝗶𝗲𝘀.

Christine Cocanour, MD, FACS, professor of clinical surgery at UC Davis Health in Sacramento, California, and chair of the ACS Committee on Trauma's Best Practices Guidelines Committee, told Medscape Medical News:

"As our committee thought about potential topics for a new best practices guideline, spine injury was a leading topic, because although it is relatively rare, it can have devastating consequences, and there are a number of management areas that remain controversial."

In nonoperative injuries, the new ACS guidelines call for discontinuing bracing when possible in favor of early patient mobilization.

If surgical stabilization is needed, it should occur early in the treatment algorithm.

The gut reaction of many non–spinal trauma specialists is to immobilize the area involved rather than risk permanent spinal cord injury.

But this may often be unnecessary & may prevent much-needed early mobilization.

Although CCS tends to affect those over 50; athletes are vulnerable to this syndrome.

CCS tends to be seen in athletes who present with not only hyperextension injuries to their neck but associated ruptured disc(s) that cause compression of the spinal cord.

Therefore, persons with arthritis changes in the bones of the neck are predisposed to CCS if any forceful extension of the neck occurs e.g. car accident.

In such situations, the canal through which the spinal cord travels can be narrow & the spinal cord can be squeezed. There is usually no obvious break or fracture in the bones of the neck and spine may be stable.

When the spinal cord is squashed, bruising, bleeding & swelling can occur, particularly in the center or central portion of the spinal cord.

Since the spinal cord is organized with the nerves that control the movement of the arms in the center & the nerves going to the legs more toward the outside, the arms are affected more than the legs in this situation.

Currently, there is no available medical treatment that can restore discs to their original condition. It’s also impossible to stop further disc disintegration, though you may be able to slow it by avoiding activities that strain your back. If the pain is not severe, home care, such as applying heat or ice to the affected area and taking an over-the-counter anti-inflammatory medication like ibuprofen, may be sufficient to relieve pain. More serious discomfort may require medical treatment, including steroid injections.
