Bruises without trauma!

45yrs old female presented with gum bleeding while chewing betel leaves for 3days. At first she ignored later she developed spontaneous bruising and petechial rash over her both limbs and abdomen. She had no history of fever, wt loss, joint pain or recent drug intake.

She is diabetic and hypothyroid for 4years.

On examination:
Multiple petechiae and ecchymoses over both limbs, abdomen.
No lymphadenopathy.
No hepatosplenomegaly.
Vitals stable.

Investigations reveals:
Hemoglobin: 10.4 g/dL
WBC count: 8,910/cumm

*Platelet count: 3,500/cumm

🔴 A Very Typical Yet Easily Missed Case!

A 35yrs old male farmer presented with,
▪️ High-grade fever with chills for 7 days
▪️ Subconjunctival hemorrhage
▪️ Decreased urine output with dark-colored urine.
He had history of bathing in pond water.

Investigations revealed:
• CBC: Neutrophilic leukocytosis
• Serum Creatinine: 2.1 mg/dL
• Urine R/M/E: Protein (+), Red cell casts +
