art of Healthy living : & its , of , , , while - , while - , , & - Every now and then, I feel the need for a deep breath or yawn, which becomes challenging. When I stand up, sit up straight, or lean slightly forward, it often becomes easier to satisfy. No hyperventilation and not gasping. Started in late 2018. has left and returned at times. My exercise activity continues to be strong- 45-60 minutes of cardio several times weekly- no significant impact. Can you tell me the causes of my excessive yawning? Why do my eyes water while yawning and why do I keep yawning while I am sick?

Thank you for the query. I can understand your concern. According to your statement, you have been suffering from needing for a deep breath or yawn excessively.

Yawning is a common reflex where your mouth becomes opened, take in a deep breath and then exhale rapidly. After yawning, generally you feel more relaxed.

Yawning may cool the brain. A yawn causes the jaw to stretch out, increasing blood flow in the face and neck. The large inhale and rapid heartbeat caused by the yawn also causes blood and spinal fluid to cycle through the body faster. This whole process may be a way to cool down a brain which has gotten too hot.

Studies suggest that yawning improves mental alertness and efficiency. Yawning helps with deep relaxation and reduces stress. Studies have found that yawning increases when people are engaged in different mental tasks.

When you yawn, your mouth becomes opened as well as you have to take in a deep and long breath that is accompanied by a sigh occasionally. Usually, yawning is the normal response of the body for drowsiness or fatigue. Yawning may happen when you become sleepy or tired. Again, medications that are used in the treatment of depression, anxiety or allergies may cause yawning excessively. Yawning may occur as an automatic response from your body during positional changes.

The vagus nerve is situated on neck, chest and intestines. It regulates the heart and blood vessels. When it is stimulated, you start to yawn excessively. You can also feel nauseous, lightheaded and break out into a cold sweat.

Some psychological disorders such as anxiety, stress, depression, mood swings, irritability, malaise, lethargy or lack of energy due to any reasons may cause yawning excessively.

Again, underlying health conditions such as sleep disorders like sleep apnea or changes in sleep pattern due to shifting duty, thyroid hormone dysfunction, electrolytes imbalance, cardiovascular diseases, neurological disorders like stoke, seizures or multiple sclerosis, respiratory diseases like bronchitis or emphysema etc also may cause yawning excessively.

Treatment depends on causes. You can undergo some investigations like CBC, thyroid function tests, serum electrolytes, ECG, ECHO, MRI of brain etc to find out the exact causes for your deep breath or yawn excessively and then treat them accordingly.

If you feel the requirement to yawn, drink a sip of ice water. Snack on cool foods like refrigerated watermelon or cucumber, whenever you want to avoid yawning. Keep your environment cool. Because, one reason we yawn when the brain's temperature too warm, it will help to avoid sitting in a hot environment.

Again, your eyes probably water when you yawn because your facial muscles tighten up and your eyes get all scrunched up, causing any excess tears to spill out. If your eyes water a lot when you yawn, it could be due to dry eyes, allergies or other conditions which affect tear production.

Take care. In case of any other query ask me.

art of Healthy living : or of ***s ***s, , , , , , , - My ge****ls have something crazy going on. Bumps on the head of my p***s and in the fo****in area. That area is also swollen, redness, soreness and puffy. I am not able to pull back my tight and painful fo****in. What should I do now? Please help me. (Picture has been removed for patient's safety and identity)

Thank you for the query. I can understand your concern. According to your statement, you have been suffering from bumps on the head of p***s and the fo****in area with swelling, redness, soreness, puffiness and unable to pull back the tight fo****in.

According to the picture that is provided here, you may suffer from Balanitis which is the inflammation of the g***s p***s caused by infections like fungal, bacterial or viral, poor hygiene, allergies or irritants, s*xually transmitted infections, diabetes mellitus, phimosis etc.

Poor hygiene is one of the most common causes of balanitis. The fo****in on uncircumcised men is a damp as well as warm area. If the skin is not washed correctly, yeast and bacteria may develop between the layers of skin. Yeast, bacteria or virus can produce an infection that leads to the inflammation of the fo****in and g***s p***s.

Clinical features are redness and swelling of g***s p***s, itching, soreness, tight and shiny skin on the g***s, unpleasant smell, tight fo****in that can't be pulled back, painful sensation during micturition, pe**le discharge, bleeding around the fo****in or swollen glands near the p***s etc.

Other reasons that causes such type of clinical features which may be resulted from pearly pe**le papules, fordyce spots, s*xually transmitted infections like ge***al herpes, syphilis, ge***al warts or molluscum contagiosum etc, lichen planus etc.

You should undergo some investigations like Urine R/M/E & C/S, urethral or prostatic swab for C/S, Biopsy from the bumps of head of p***s etc to find out the exact causes for your present sufferings.

As I have already mentioned, treatment for balanitis depends on the underlying cause, it begins simply with cleaning the head of the g***s p***s and fo****in. Once the cause of balanitis is diagnosed, self care at home may be all that is required to treat the condition.

Most cases of balanitis are easily treated with good hygiene, antifungal or antibacterial or antiviral topical creams or ointments and mild steroid creams like hydrocortisone. People are advised to clean the p***s daily with lukewarm water and to dry it gently to improve hygiene. Patients should avoid using soaps, bubble bath or shampoo on their ge****ls and dry under the fo****in after urinating.

Without treatment, balanitis can produce some serious complications such as narrowing the opening of through which urine leaves the body or urethral strictures, urinary retention, blackflow of urine toward the kidneys or vesicoureteral reflux, painful retraction of the fo****in, lack of blood flow to the p***s etc. If it gets worse, you might need to be circumcised to treat it.

To ensure your balanitis doesn't come back, you can adopt some good habits such as wash and dry the area completely, don't have s*x while you are having balanitis symptoms, use condoms which fit correctly and are formed with materials which will be suitable on your skin, wash your hands before urinating, if s*x triggers your symptoms, clean your p***s after you have it.

Take care. Hope I have answered your question. Let me know if I can assist you further.

art of Healthy living : or & its , , , , , of , - New rash all over chest and back. Appeared in last 3 months. Slightly pinker than rest of skin, not painful or itchy.

Thank you for the query. I can understand your concern. According to your statement, you have been suffering from rashes all over chest and back for the last 3 months that are slightly pinker than rest of the skin but not painful or itchy.

Rashes on chest and back may result from various possible reasons such as allergens or irritants, Infections like fungal, autoimmune disorders, Lyme disease, medications, stress and many more.

According to the pictures that are provided here, you may suffer from tenia versicolor which is a common skin Infection due to overgrowth of a skin surface yeast. This yeast usually lives in the pores of the skin and thrives in oily areas of the body like upper chest, back or neck.

Tinea versicolor can be scattered over the chest as well as back and appear as tan to pink spots on light or fair skin like yours, while tinea versicolor can be light or dark on dark skin.

The yeast prevents the affected skin from tanning normally by growing slowly. The rest of the skin tans normally but the yeast affected pink spots become more noticeable due to lack of tanning.

Tinea versicolor usually causes no symptoms or a few symptoms. Sometimes, it may cause mild itching that can become more intense when a person gets hot.

Tinea versicolor is also known as tinea pityriasis and may present with white, pink, salmon-coloured, tan or brown patches of skin discoloration ranging in size from 1-3 cm generally on chest, back, neck and upper arms that can appear lighter or darker than usual, mild itching, scaling etc.

This yeast can stay in small numbers on anyone's skin. Anyone may develop an overgrowth of yeast during the summer season, because the yeast can increase due to high temperature and humidity. The overgrowth of the yeast on the skin can prevent the normal pigmentation process resulting in tan to pink spots.

A number of factors may trigger this yeast growth such as oily skin, hormonal changes, weakened immune system besides hot and humid weather.

Examination of the fine scales scraped from the pinkish spots under microscope for the presence of the yeast can confirm diagnosis of tinea versicolor.

Antifungal shampoos containing Ketoconazole or selenium sulfide are the first treatment recommended for tinea versicolor. Antifungal topical creams or lotions like Clotrimazole or Miconazole can be used for small affected area. If your tinea versicolor doesn't respond to these topical treatment, then oral antifungal may be needed to kill the fungus.

Even after successful treatment, your skin colour may remain uneven for several weeks or even months. Again, the yeast infection can come back in hot and humid weather. In persistent cases, you may require to use a medication once or twice a month to prevent the infection from recurring.

Take care. In case of any other query ask me.

art of Healthy living : or & its , , , , , , & its & - I have been struggling with restless legs for years. I took Pexola at increasing dosages (ended with 1mg) till July 2019. I then started to take Requip XL 2mg twice a day - as my RLS was giving me a hard time during day time as well. It worked very well but now my legs are acting up again BIG time! I have no trouble falling asleep but I wake up at about 12:00 pm and have difficulty falling asleep again. I started jogging about three months ago - lost about 14 kg, I eat a healthy diet and limit my alcohol and caffiene intake. I have no other health issues. I am 32 years old, 1.65 m, 55 kg. My other medication: Dormanoct 2mg, Cilift 20mg, Prexum 5mg. I just want something that can help me with a good nights sleep.

Thank you for the query. I can understand your concern. According to your statement, you have been suffering from restless leg syndrome and insomnia.

Restless Leg Syndrome causes unpleasant or uncomfortable sensation in the legs and an irresistible urge to move them. It usually occurs in the evening or night time hours when someone is sitting or lying down. Moving eases the unpleasant sensation occasionally.

The main presentation of restless leg syndrome or also known as Willis Ekbom disease is an overwhelming urge to move the legs. It can also cause an unpleasant crawling or creeping sensation in the feet, calves and thighs. Occasionally, the arms can be affected too.

Restless leg syndrome is also associated with involuntary jerking of the legs and arms, known as periodic limb movements in sleep or PLMS.

Some people have the symptoms of Restless leg syndrome occasionally, while others have them every and the symptoms may vary from mild to severe. In severe cases, Restless leg syndrome can disrupt a person's daily activities like sleeping problems.

In the majority of cases there is no obvious causes of restless leg syndrome. Some neurologists trust the symptoms of Restless leg syndrome may have something to do with how the body handles a chemical messenger known as dopamine.

Dopamine is involved in controlling muscle movement and may be responsible for the involuntary leg movements associated with Restless leg syndrome.

In some cases it is caused by an underlying health condition like Iron deficiency anaemia, Parkinson's disease, renal disease, diabetes mellitus, peripheral neuropathy, chronic diseases, vitamin deficiencies like vitamin B complex, nutritional deficiencies like calcium, magnesium or Vitamin D, dehydration etc.

Some investigations like CBC, serum ferritin level, serum creatinine, RBS, vitamin B and folate test, serum calcium and magnesium levels, vitamin D test, serum electrolytes etc should be undergone to find out the exact causes for the Restless leg syndrome and then treat them accordingly.

Again, there is also a link between Restless leg syndrome and pregnancy. About 1 in 5 pregnant women will experience symptoms in the last 3 months of their pregnancy and Restless leg syndrome usually disappears after the delivery. But the exact cause is still unknown.

It is a common condition which can affect anyone at any point in their life. But women are twice as likely to develop Restless leg syndrome as men. It also more common in middle age, although the symptoms can happen at any age.

If Restless Leg Syndrome strikes when you are asleep, take a walk for 5 minutes. Usually just getting out of bed quickly can make symptoms disappear. When symptoms develop in bed, try rubbing or lightly messaging your legs. Avoid spending more than 20 minutes awake in bed with restless leg syndrome.

Some other measures can be taken to subside Restless leg syndrome such as cut out caffeine as well as alcohol, avoid smoking, take a hot bath to relax the muscles, apply ice packs to the legs, don't eat a big meal right before bed, practice mediation or yoga to reduce symptoms, take daily walks etc.

The choice of medication should be based on patient's symptoms and requirements-

▪︎ Low dose dopamine agonists like Ropinirole are first line treatment for daily symptoms of Restless leg syndrome.

• Dopamine precursors like Levodopa can be given if dopamine agonists are not tolerated or if medicine is only required intermittently.

Anticonvulsants like Gabapentin can be considered if treatment with dopaminergic medications has failed or is contraindicated or where symptoms are painful.

Chronic insomnia can be resulted from anxiety, stress, depression, anger, worry, grief, trauma etc. Again having difficulty sleeping can also make anxiety, depression, stress symptoms worse.

For your insomnia you can follow a sleep schedule. You should try to sleep at a certain time regularly. Keep your bedtime and wake time consistent from day to day including on holidays.

You can take some medications for sleeping like Eszopiclone (Lunesta), Zolpidem (Ambien), Midazolam, Clonazepam etc at bedtime for a week.

Take care. In case of any other query knock me and I will try to reply you further.

art of Healthy living : & its , , # Diagnosis, - Sorry to bother you drs. You must be very busy. It's just I have severe chills that I needed to wear thick sweaters and jackets at home without a/c or fan. I dont feel the heat. And just cold water or slight cold breeze in the morning makes my head frozen or close to shutting down. It's getting worse. It always starts with the nose and then my head and the body follows. Sometimes I have to sleep to make the symptoms less severe. I'm not under any medications nor do I have any medical condition. I'm female, Malay, and 23 years old. Previous history of the same issue: Previously it was much mild. Now it becomes severe. Please help me.

Thank you for the query. I can understand your concern. According to your statement, you have been suffering from severe chills that you are needed to wear thick warm clothes at home without a.c. or fan in summer now.

Chills are the sensation of feeling cold. Chills are caused by rapid muscle contraction and relaxation. They are the body's way of producing heat when it feels cold. They often occur alongside a fever which is when a person's temperature is higher than normal.

However a person can also experience chills without a fever like you and there can be many different reasons for this. Possible causes are hypothyroidism, bacterial or viral gastroenteritis, flu, meningitis, sinusitis, pneumonia, strep. throat, urinary tract infections, kidney stones, hypoglycemia, malaria, anemia, vitamin B 12 etc.

Hypothyroidism is when a person's thyroid gland doesn't produce enough of the hormone that regulates their metabolism. Some people like you are more sensitive to the cold and may feel chills more frequently. Any type of viral or bacterial infections sometimes produce chills before a person gets a fever.

Chill is a classic symptom of Malaria.it can have 3stages:▪a cold stage where a person experiences chills ▪a hot stage where they have a high temperature ▪a sweating stage. Anaemia may develop due to poor diet, age, chronic illness, medications, gynaecological diseases etc.

Most common symptoms include waking up tired, looking pale and always feeling cold with chills. Hypothermia can be resulted from dropping body temperature below normal range due to any causes.

Hypotherma can be dangerous especially the further your body temperature falls. If temperature falls below 96 degrees you should seek medical care immediately. So you should monitor it body temperature with a thermometer during feeling chills.

Chills develop in the mild and moderate stages of hypoglycemia. Hypoglycemia is a ccondition in which your blood sugar or glucose is lower than normal, usually under 70 mg/dL. It can occur in people with diabetes when they continue to take their usual doses of insulin, sulfonylureas or meglitinides.

Again, hypoglycemia can result from not eating enough carbohydrates, skipping meals or not eating frequently enough, increasing physical activity, consuming too much alcohol without eating enough, sick with the flu or other illnesses. Feeling cold all the time or most of the time is a symptom of malnutrition. Malnutrition is when your body is starving for nutrients and can't function properly.

Malnutrition has many reasons such are not consuming enough nutrients, not consuming enough food, not absorbing the nutrients and vitamins from food etc. Without the necessary nutrients it requires, your body can't maintain a healthy and comfortable body temperature, so you will suffer from chills.

You can do some investments like CBC with PBF, Thyroid function test(t3, t4, tsh), RBS, Ultrasound, serum vitamin B12 assay etc. Treatment depends on causes. So we should find out the exact causes for your sufferings and then treat them accordingly. Again, other causes like dehydration, anxiety, panic attacks may cause chills too.

When your body doesn't have enough fluids, it's hard to maintain a regular body temperature and this can lead to hypertherma and fever like symptoms including chills. Again anxiety is more often associated with feeling sweaty than feeling cold but sometimes it can cause a chilly feeling as well. And if you have panic attacks you might experience full body chills according to the National institute of Mental Health (NIMH).

In order to cope up with your Chills you can do as following- • Drink lots of fluids including oral rehydration saline and plenty of rest. • Sponge with lukewarm water. • Don't bundle up in blankets or use a/c or high speed fans. • Enjoy a warm soup. • Drink warm beverages. • Use a humidifier. • Eat red meats, sweet potatoes, bananas, oats, butternut squash, drink ginger tea etc.

Please inform the results after undergoing the investigations and then I will be able to help you further. Take care. Hope I have answered your question. Let me know know if I can assist you further.
