Evaluation of unexplained radiological opacities with mediastinal lymphadenopathy in 80 years old male patient.
Attempt to obtain tissue by FNAC was not possible as lesion was centrally located.

Bronchoscopy shows small nodular infiltration along the Right main bronchial tree with extensive inflammation and compression.

Biopsy taken and sent for HPE.

বর্তমানে চেম্বার করছি বক্ষব্যাধি বিশেষজ্ঞ হিসেবে শুধুমাত্র Al Haramain Hospital এ।
প্রতিদিন (শনিবার থেকে বৃহস্পতিবার )
বিকাল ৪ টা থেকে রাত ৯ টা পর্যন্ত।

তাছাড়া সপ্তাহের প্রতিদিন ক্রিটিক্যাল কেয়ার ডিপার্টমেন্ট (আই.সি.ইউ) ও ইনডোরে ভর্তি রোগি দেখছি। সাথে
রেসপিরেটরি ইন্টারভেনশান( Bronchoscopy) সার্ভিস চালু আছে।

A 60 years old Male was suffering from Resp. distress with H/O PTB , comes at Al Haramain Hospital from JRRMCH for further evaluation of airways.

xray showing left lung collpase and hyperinflation of right lung.

Bronchoscopy was done which shows extreme narrowing with some inflammed ulcerated mucosa possibly due extensive fibrosis of left lung due to TB.

Biposy taken for HPE along with Broncho-alveolar lavage for gene xpert.

Bleeding was there and proper haemostasis was done during procedure.

A 30 years old female was admitted at NEMCH, suffering from dry cough for 2 months with recurrent H/O high grade fever. Despite of I/V antibioics her condition was not improving.

Empirically Anti-TB was started as there was no evidence. But after 3 weeks of anti- TB ,she develop a new radiological shadow in Cxr.

A New Right middle lobe collapse was suspected at CT Chest.

So she was referred to Al Haramain Hospital from NEMCH for further evaluation.

She had undergone Bronchoscopy. There was no occluding lesion with patent distal airways EXCEPT some small nodular lesion over opening of Right middle lobe bronchus.

Biopsy was Taken from lesion and sent for HPE.
Broncho-alveolar lavage was also taken

Biopsy shows Bronchial mucosa suggestive of Granulomatous inflammation (non caseating)
D/D can be : NTM / fungal infection, sarcoidosis etc.

BAL for gene xpert shows MTB detected (very low burden).

A 70 years old women from SWMCH ,was referred to Al Haramain Hospital for evaluation of airway as she is suffering from
"unresolved Pneumonia"

Her Endobronchial tree was normal.

Important Indication of Bronchoscopy :

1. For evaluation of any unexplained symptoms like any unexplained Cough,SOB,wheezing,stridor or hoarseness of voice.

2. Unexplained Radiological opacities
3. Unresolved pneumonia/ recurrent pneumonia
4.Mediastinal or hilar lymohadenopathy.
5.Lung abscess specially in smoker and older adult.
6.Lung collapse due to any cause
7. Suspected foreign body impaction.

18 years old male Comes to my chamber at Al Haramain Hospital with H/O dry cough for 2months with single episode of fever 1 month back.

CxR shows unexplained opacities involving Middle zone of right lung.

Sputum for gene xpert was negative as there was no sufficient sputum production.

Bronchoscopy was done and BAL( Broncho-alveolar lavage was taken) sent for gene xpert which become POSITIVE.

Usually, this kind of scenario we can start Anti-TB drug empirically on basis of radiological evidence. but doing a bronchoscopy in such patients provides you conclusive evidence and more confidence on starting anti-Tb.
