I discussed the nuanced role of in managing chronic urticaria (CU)—a topic that’s often surrounded by misconceptions.
✅ Key Highlights:
1. Immunologic food allergies are extremely rare as causes of CU, but may play a role in specific cases, such as:
• Patients with a history of tick bites (linked to delayed meat allergy)
• Those consuming raw, marinated, or smoked fish
2. Non-immunologic food reactions are more frequently reported:
• These include sensitivities to natural foods like 🍅tomatoes, 🐟seafood, 🌿herbs, and ➕food additives.
• While the exact mechanisms remain unclear, the link to symptom worsening is recognized.
3. Elimination diets show promise:
• Clinical trials show significant symptom improvement within 3 weeks of eliminating potential triggers.
• The Oral Provocation Test (OPT) confirms that reintroduction of certain foods can worsen CU symptoms.
4. Practical Recommendations:
• First-line treatment remains oral antihistamines.
• Dietary education is valuable for selected patients, particularly those with:
• Suspected delayed meat allergy
• Reactions to specific foods or additives
• Risk factors for celiac disease or vitamin D ☀️
• Consider a 3-week elimination diet focused on pseudoallergens and high-histamine foods, guided by a food diary.
Caution:
Elimination diets aren’t for everyone and should be:
• Short-term or supervised by a nutritionist
• Based on evidence-backed approaches
