Myelomeningocele (Spina Bifida)

What is it?

Myelomeningocele is a birth defect where part of the spine and spinal cord don’t close properly in early pregnancy. This leaves nerves exposed in a sac on the baby’s back, which can cause weakness, bladder/bowel problems, and sometimes fluid build-up in the brain (hydrocephalus).

Why does it happen?

It occurs when the neural tube (which becomes the brain and spinal cord) doesn’t fully close in the first month of pregnancy.

Common Signs
• Sac on the back at birth
• Weakness/paralysis in the legs
• Bladder or bowel control problems
• Possible foot, hip, or spine deformities
• Hydrocephalus (in many babies)

Treatment & Care

Care involves a team approach:
• Surgery – Close the opening soon after birth (sometimes before birth in special centres).
• Urology – Regular bladder checks; catheterisation and medicines may be needed.
• Physiotherapy – Maintain strength, mobility, and prevent deformities; braces or walking aids may help.
• Hydrocephalus management – May need a shunt or endoscopic surgery.

Living with Myelomeningocele

With prompt surgery, good bladder/kidney care, and regular physio, many children grow up to attend school, work, and live active lives. Some walk independently, others use wheelchairs. Lifelong follow-up helps protect health and independence.

SPINE SURGERY: FACTS vs MYTHS

Let’s bust some common myths — your back will thank you!

🔹 MYTH 1: “Once you have back surgery, you’ll always need another one.”

✅ FACT: Not true! Most people do well with just one well-planned surgery — and go on to live active, pain-free lives. The key? The right diagnosis, the right team, and good follow-up care.

🔹 MYTH 2: “If you have back pain, you’ll need surgery.”

✅ FACT: Nope! Surgery is rarely the first step. Most back pain gets better with physio, exercise, or other non-surgical treatments. We only recommend surgery when it’s truly needed.

🔹 MYTH 3: “Spine surgery means a long stay in hospital.”

✅ FACT: Not anymore! Many spine procedures use smaller cuts and modern tools. Some patients are even up and walking the same day — and home soon after!

REMEMBER:

No two spines are the same — and neither are treatment plans.

✅ What is Back Pain?

Back pain is a common reason for doctor visits. It may come on suddenly or develop gradually, and affect the neck, upper, or lower back.

🔍 Common Causes
• Muscle or ligament strain
• Herniated (slipped) disc
• Spine degeneration (arthritis)
• Poor posture or prolonged sitting
• Injury or trauma
• Infections or tumors (rare)

⚠️ When to Seek Urgent Help

See a doctor immediately if you experience:
• Numbness or tingling in legs/buttocks
• Leg weakness
• Loss of bladder or bowel control
• Fever or unexplained weight loss
• Severe pain after trauma

🧪 Diagnosis

Your doctor may:
• Review your medical history and examine your spine
• Order imaging (X-ray, MRI, or CT scan)
• Perform nerve tests if needed

💊 Treatment Options

Non-Surgical:
• Pain relievers and anti-inflammatories
• Muscle relaxants
• Physiotherapy and posture correction
• Weight and activity adjustments
• Nerve blocks or epidural injections

Surgical:
Recommended if:
• A disc compresses a nerve
• There’s spine instability or deformity
• Conservative care fails

Procedures may include:
• Discectomy
• Laminectomy
• Spinal fusion
• Minimally invasive spine surgery

🏃‍♂️ Self-Care Tips
• Keep active – avoid prolonged bed rest
• Strengthen your core
• Maintain good posture
• Avoid sudden or heavy lifting
• Use a supportive mattress

What Are CNS Infections?

Central Nervous System (CNS) infections affect the brain and spinal cord. They can be life-threatening if not treated quickly.
Causes include bacteria, viruses, or fungi, often leading to pus or fluid build-up that increases dangerous pressure inside the skull or spine.

Common conditions include:
• Brain Abscess
• Subdural Empyema (pus trapped around the brain)
• Spinal Epidural Abscess & Spondylodiscitis
• Postoperative CNS Infections

🚨 Symptoms to Watch For

Seek urgent medical care if you notice:
• Severe headache
• Fever and chills
• Neck stiffness
• Seizures
• Weakness, numbness, or confusion
• Back pain or difficulty walking (with spinal infections)

🧪 How We Diagnose

We use advanced tests such as:
• MRI & CT scans
• Blood tests & cultures
• Biopsy or abscess aspiration

💧 What is Hydrocephalus?
Hydrocephalus is a buildup of cerebrospinal fluid (CSF) in the brain, causing increased pressure. It can affect infants, children, and adults.

🔎 Causes
• Birth malformations
• Brain tumors or cysts
• Infections (e.g. meningitis)
• Bleeding in the brain
• Normal Pressure Hydrocephalus (in older adults)

⚠️ Symptoms to Watch For
👶 Infants: enlarged head, bulging fontanelle, vomiting, delayed development
👦 Children/Adults: headaches, blurred vision, walking/balance problems, memory or personality changes
👵 Older Adults (NPH): shuffling walk, incontinence, memory decline

🧪 Diagnosis
• CT or MRI scans
• Neurological examination

🔧 Treatment Options
1️⃣ VP Shunt Surgery – diverts fluid to the abdomen
✅ Effective, but requires lifelong monitoring

2️⃣ Endoscopic Third Ventriculostomy (ETV) – creates a new CSF pathway
✅ No implant, best for obstructive hydrocephalus

⏱️ After Surgery
• Hospital stay: 1–3 days
• Recovery: usually within weeks
• Follow-up: essential for long-term success

🚨 Seek urgent help if you notice:
Severe headache • Vomiting • Confusion • Swelling/redness along shunt • Return of symptoms

Endoscopic Pituitary Surgery

1. What is endoscopic pituitary surgery?

Answer:
Endoscopic pituitary surgery is a minimally invasive technique used to remove pituitary tumors through the nose using a thin camera called an endoscope. It avoids external incisions and generally allows for quicker recovery and fewer complications.

2. What are the symptoms of a pituitary tumor?

Answer:
Symptoms can include headaches, vision disturbances, fatigue, weight changes, menstrual irregularities, low libido, and hormonal imbalances. Some tumors are found incidentally or during evaluation for hormone-related issues.

3. How long is the recovery from endoscopic pituitary surgery?

Answer:
Most patients stay in the hospital for 1–2 days and resume light activities within a week. Full recovery can take several weeks. We will closely monitor your healing, hormone levels, and post-op imaging.

4. Will I need hormone replacement after pituitary surgery?

Answer:
In some cases, especially if the surgery impacts hormone-producing areas of the pituitary. Our team works closely with endocrinologists to assess hormone function and provide appropriate therapy if needed.

5. Is endoscopic pituitary surgery safe?

Answer:
Yes. When performed by an experienced team, it is very safe. Potential risks like cerebrospinal fluid leaks, temporary hormonal shifts, or nasal bleeding are uncommon and usually well-managed.
