Central Brampton Family Health Team is looking for a Registered Dietitian, Clinical Diabetes Specialist to join our collaborative care team in Brampton.
Salary: CA$72,107 to CA$76,150 per year
Job Type: Full time
Work Location: Hybrid remote, Brampton, ON
In this role, you will deliver evidence based nutrition care across a broad range of health conditions, with a strong focus on diabetes management. You will conduct comprehensive nutrition assessments, develop personalized care plans, lead group education sessions, and support patients through meaningful lifestyle change. From blood glucose monitoring and medication adherence to weight management and label reading, you will empower patients with the tools and confidence to manage their health effectively.
We are seeking a Registered Dietitian in good standing with the College of Dietitians of Ontario, with experience supporting patients with diabetes or other chronic conditions in a clinical setting. Strong communication skills, interdisciplinary collaboration, and a passion for patient centered care are essential.
