Dr. Rosa Lee의 가정의 (Family Doctor) 등록를 위해 간단한 방문 신청를 진행하게 되었습니다.

이 과정은 필요성/순차적으로 환자분들은 받기 위한 절차이며 방문과정 자체는 정식 가정의 등록의 확정이 아님을 알려드립니다.

방문 접수 이후 등록이 되신 분들께 신속하게 연락을 드리되, 연락을 받지 못하신 분들은 대기자명단에 올려드려 추후공지 또는 의사의 지시에 따라 연락을 드리겠습니다.

접수 당일 Personal Health Number (PHN)을 꼭 지참해주시고 접수하는 본인이 직접 방문하셔야 하는점 알려드립니다.

신청을 원하시는 분은 아래 원하시는 시간대를 선택해 클리닉을 방문하여 간단한 질문지 작성과 접수를 하시면 됩니다 (약 5분 소요).

다른 환자분들을 위해 꼭 신청하신 시간대에 작성 시간을 염두해 두시고 오셔야 하는 점 유의 부탁드립니다.

*이미 가정의가 있는 신청서는 접수되지 않습니다.

SIGN UP for LAMURIA Medical Clinic FAMILY DOCTOR APPLICATION DROP IN

This form is to sign up for a time slot for an in-personal drop-in application for those wishing to register Dr. Rosa Lee as their family doctor.

Your visit will take approximately 5 minutes to fill out the application form and register at the front desk.

You cannot apply for another individual - all individuals applying must visit in person themselves.

Please have your Personal Health Number (PHN) ready for your visit.

Those who have been confirmed will be contacted promptly after the assessment.

Those who have not been confirmed will be put on a waitlist, and we will reach out through future announcement or under the request of the doctor.

In courtesy of other patients, please come ONLY during your selected time slot with enough time to complete the application process.

*DO NOT sign up if you already have a family doctor - applications with pre-existing family doctors will not be considered
