The suffix “itis” in medicine describes the inflammation of a specific body part. However plantar fasciitis isn’t what it seems. Just by the name, one would think that there’s an inflammation in the fascia of the foot, but that is not the cause. Typically the two major muscles that cause this painful issue are gastrocnemius and soleus. With plantar fasciitis it’s an irritation caused by these tight muscles. When these two calf muscles are tight, they can cause severe pain at the distal attachment of their shared tendon. This causes severe pain at the heel, especially when getting up in the morning and upon long periods of standing. Massage therapy and medial arch support can help, but with this condition, I will provide specific exercises to lengthen both the gastrocnemius and the soleus. If done correctly, this typically helps, but when it doesn’t I add stretches for other muscles in the legs and hips.

“You don’t treat MVA’s?“. Actually I do; I just don’t bill for them. Let me explain. If someone comes into a clinic to be treated for an MVA, and I bill that patient the full amount, he can send it to his insurance company and will get his percentage back usually within days. So say his insurance company pays 70% for MVA’s and he’s billed $100. He pays $100 then can submit the full amount to his insurance company, and will get $70 back usually in less than a week. Alternatively, if I bill the auto company the $70 and charge the patient the $30 left over, it takes much longer for the auto insurance company to pay what’s coming to me. I’m waiting for that $70 for usually three months, because insurance companies take significantly longer to pay the therapist. So each time a person comes in for an MVA treatment, that $70 is withheld from the therapist for three months from the date of the treatment. This is just how auto insurance companies work; they don’t pay the therapist in a timely manner, but will pay the patient in a timely manner. That is why most therapists will not bill MVA auto insurance companies.

Each therapist treats in his or her own way. Some are more relaxational, whereas others are more therapeutic and injury based, some provide a lighter pressure and others provide a deep pressure. For instance, I get asked for deep pressure (what most people call deep tissue) quite often, but I don’t do it anymore partly because I’ve been injured providing deep pressure many times. It’s best to ask for somebody who does deeper pressure when you schedule your appointment. You should check to find out what type of therapist you’re booking with so that you’re getting what you’d like and also so that you’re not asking the therapist to do something that he or she is not able to do.

You should always read a contract before signing it. Our membership contract is very detailed and discusses PAD, and your obligations. Some of those obligations include a PAD being charged if you miss a month’s appointment, being charged for not showing for an appointment and 24 hour notice of not being able to come in for your appointment m. The time that you book is set aside specifically for you, so if you are not there, it is a waste of therapist time. It is very common in healthcare to charge a no-show fee, so make sure you read and understand a contract before you sign it.

Sometimes patients come in and tell me they just want a massage. That doesn’t tell me anything; this is why RMT’s ask questions. Do you have pain? Stress? Dysfunction? Were you in an accident? Did you have a fall? We don’t go anywhere else and ask for “just” something, so don’t diminish what your RMT does.

Hot or cold: which one should you use? Many patients come in having used the wrong one. Most people use heat because it feels good, but it’s not always the right thing to do. If you have injured your body, you should use cold, 10 minutes on then 10 minutes off. If, however, you have tight muscles, you will want to use heat, 15 minutes on top of clothing. One exception to these rules is headaches , and it depends on what kind of headache you have. If you have a tension headache , it comes from tight muscles. Then, we will put heat on the neck, again for 15 minutes. If you have a migraine on the other hand, put cold to your head for 10 minutes while heating your feet.

There is a television advertisement on lately that purports that if you can’t move your neck you should go in for a pillow fitting to get new ones. If you wake up and can’t turn your head, it is usually because of a specific muscle in the back of your neck, that goes into your shoulder. This muscle will shorten overnight if your head and your shoulder are close together while you sleep. Your brain puts you in the positions it wants you to be in throughout the night; there’s nothing you can do to prevent it. But if I were to assess you and find that this muscle is short, I could teach you how to stretch it, so that you can maintain its length. This way, it should feel better and you should have more range of motion. No need to buy new pillows that don’t work.

Assessment is a legally-required and important part of your massage treatment. Without assessment we are treating with only half of the information we should have so we can’t be as effective in your care. It is never a waste of time so it is best to have patience and answer our questions without assuming you know what is causing your discomfort or pain.

If you want the bolster, you need to ask reception to ensure they schedule you with a therapist who uses one.

To avoid frustration, you need to be able to communicate effectively with your therapist. We ask questions on purpose, to get us the information that we need but we are not mind readers. If you only want your mid back done, but ask for back, neck and shoulders, we are going to treat the entire back, neck and shoulders. You can communicate with us during treatment; if there’s something that needs to change, ask away. Two rules of thumb: never assume and always be polite.

I noticed someone sitting in an awkward position today and at a moments glance, I told him not to sit thst way, as it’s very hard on his joints. Sitting on a chair, knees bent, ankles crossed underneath his legs… It may sound comfortable, but what did I see ? Most notably, the pelvis was out of line which would create a curve in the spine, causing lower back pain. The neck was flexed to the side and the shoulders were uneven, both causing neck pain. Both femurs were rotated, but oppositely - one laterally and one medially. There was rotation in the lower legs as well and too much load being put on the ankles. All this, just by sitting in a poor position. Do yourself a favour and sit correctly!

Keep a couple things in mind: now is a great time to use up those benefits you still have (check with your insurance first) but because of that, it’s getting increasingly difficult to book. As well, you may want to consider a gift certificate for your soon-to-arrive holiday shopping!
