Essential clinical assessment tools - can you name them all?

Our assessments are much more than simply taking vital signs. We provide head-to-toe assessments, psychosocial and emotional/psychological assessments, and more. We use this information to assess health care needs, determine a nursing diagnosis, plan care, implement interventions, and evaluate the effectiveness of care.
Critical thinking plays a big role in the nursing process and goes beyond basic problem solving.
CRITICAL THINKING -
It's at the center of clinical reasoning and clinical judgment. It combines logic, intuition, and creativity.
Nurses need critical thinking for:
• Decisions required to draw on knowledge from other disciplines;
• Dealing with change in a stressful environment;
• Making frequent and important decisions involving health, safet, and welfare of patients;
• Includes a holistic application as well.
