It is our final day hosting the Health Survey at the community centre. Room 2 on the second floor at the end of the hall.

$40 gift cards for completed surveys! And chummus!

We’ve got you covered! In-person appointments are now available in Port Alberni at our office or in your home.

Our staff can support you in the way that works best for you:
✅ Read the questions aloud and enter your answers for you
✅ Or, simply be there to assist while you complete the survey on one of our tablets

Bonus: When you finish your survey in person, you’ll receive your gift card immediately—no waiting for it to arrive in the mail like with the online option!
