Through the North West Regional Palliative Care Program, clinical coaches are working hand-in-hand with local Family Health Teams, hospitals, LTC homes, supportive housing, and even EMS to bring the palliative approach to care closer to home. From Nipigon to Manitouwadge and beyond, these efforts are creating stronger connections, better support, and more compassionate care for those living with life-limiting illness.

Whether you’re a health care provider, community partner, or someone who simply wants to learn more, there are resources, education, and networking opportunities available to help us all work together in delivering the care people deserve.

An important and compassionate conversation on navigating a life-changing palliative diagnosis is taking place this September in Thunder Bay.

Where: Slovak Legion, Thunder Bay ON

This free community event will share 7 keys to help individuals, families, and caregivers better understand and prepare for the journey ahead — while holding onto hope.

Planned by the Regional Palliative Care Working Group, led by St. Joseph’s Care Group Regional Palliative Care Program and Lakehead University’s Centre for Education and Research on Aging and Health.

Let’s come together to learn, support, and empower one another.

Indigenous Service Providers from across the region gathered to share knowledge, resources, and support with the community. It was inspiring to see so many people engaging with services they may not have known were available — and making meaningful connections along the way.

Miigwech to everyone who came out, set up a booth, crafted, or simply stopped by to learn. Together, we’re building stronger connections and creating more awareness of the amazing Indigenous services in our community. 🧡
