Combined hard and Soft tissue grafting for both guided bone regeneration around an implant and guided TISSUE regeneration around a severely compromised canine. With out improving the keratinized tissue thickness using a connective tissue graft in a case like this can lead to a significant recession around the canine and possible complications in bone grafting. A connective tissue graft was added to maintain and improve the soft tissue coverage around this canine as well as improve the buccal tissue volume on the implant.

Step by step root coverage for the lower anterior region. This used to be an unpredictable procedure for me with tunneling due to the muscle pull in the area and excessive swelling which ultimately led to relapse or failure of coverage. Now this is something I do routinely, with modifications of the dissection planes using sliding split thickness dissection (SSD) which allows you to create a lighter flap for closure with less muscle pull. Analysis of the anatomy is also essential in establishing a realistic approach for both us and our patients. All these aspects will be covered in details in our next course with

Management of peri-implantitis
Patient presented with failing implants
In the posterior mandible. The patient did not want to replace the implants due to recently having the implants restored.
Management of peri implantitis is not only dependent on management of the bone, but the Soft Tissue element cannot be ignored!!! Especially in implants with a past history of disease. Implants were treated with implantoplasty, air abrasion, detoxification and they combined regenerative approach with ti reinforced membrane from both implants were submerged. Re-entry showing full closure over the membrane however lack of keratinized tissue present around both implants which might be detrimental to the newly regenerated bone. Apically positioned flap with split thickness bed preparation was performed, and a free gingival graft was harvested and adapted around both implants. Notice the new level of the bone/soft tissue preventing from seating and restoration. Had to be adjusted following healing.

I want to extend my deepest appreciation to everyone who has offered their condolences, support and prayers. I had/am having a very difficult time responding during this time of grief as it brings me back to square one. I did want everyone to know that I do genuinely appreciate all the love and support received for my late father and that I will be in touch soon.

Ever start a tunneling procedure and not get the release you want? What do you do? A surgeon has to adapt and part of adapting is to be versatile, sometimes you add a vista incision but its not enough, so why not combine a tunneling and vista in the anterior zone and a small vertical incision with a full split dissection to allow rotation and coverage of the defect. Old case… hopefully will be back posting soon
