Cl****al adhesions are an under-recognized finding.

They occur when the cl****al hood becomes adherent to the g***s, often in the setting of inflammation.

Common causes include:
• Lichen sclerosus (most important to rule out)
• Chronic irritation
• Low estrogen states
• Other inflammatory skin conditions

They may be associated with changes in anatomy, discomfort, or difficulty with arousal. The impact on sensation is variable and still debated.

Often missed — but important to recognize.

PRP is being marketed “down there” for things like lichen sclerosus and sexual dysfunction.

This comes up in clinic more and more, with patients asking about it.

The problem is that it sounds promising — but the evidence just isn’t there.

For lichen sclerosus, we already have treatments that are well studied and reduce long-term risk. Higher quality studies don’t support PRP use.

For sexual dysfunction, PRP hasn’t been shown in good quality studies to provide consistent benefit.

It’s also expensive, not standardized, and often marketed directly to patients.

This is where medicine and marketing start to blur — and that’s not good for patients.

I’m not a believer in selling “miracle cures.”

The safest permanent birth control isn’t what most people think.

Between vasectomy and tubal ligation, vasectomy is generally the safer option.

It’s a simpler procedure, done under local anesthesia, with a lower risk of complications and a faster recovery.

Tubal ligation is still a safe and effective option — but it’s more invasive and usually requires an operating room.

Both work well.
But they’re not equal when it comes to risk.

If you’re deciding, this is an important part of the conversation.

More data supporting HPV testing and self screening. Please advocate for it. It’s a safe option that many patients prefer to a pap. Just understand when your HPV test is positive we need to look at the cervix carefully for pre cancer/cancer and that involves a speculum (ie it feels like a pap)

Adding cytology to HPV screening (cotesting) offers little benefit over HPV testing alone for detecting cervical precancer in women 25–65, according to a large Canadian study in JAMA Network Open.

The findings come as US guidelines begin to incorporate home-based HPV testing and add to growing evidence that HPV testing alone provides comparable long-term reassurance—suggesting screening could be simplified without compromising safety. https://mdsc.pe/4dM1iuV
