We have had the privilege of serving the community of Carleton Place and surrounding areas for many years. We are sorry to inform you that our practices at McNeely Medical Clinic are closed permanently effective December 31, 2022. McNeely Medical Clinic has worked very hard over the past years to advocate for rural healthcare, and we share with great disappointment that it has become necessary to close our clinic.

Please take note of the following instructions if you are a previous patient of this clinic or are calling from another agency requiring assistance.

Dr. Asaad Al-Hindawi and Dr. Christine Tai have relocated their practice to the Westend FamilyCare Clinic. If you are an enrolled patient of Dr. Al-Hindawi or Dr. Tai, you can call the Westend FamilyCare Clinic at 613-599-3321 to transfer your records and/or schedule appointments.

If you are a previous patient of this clinic looking to have your medical records transferred, please ask your new family physician to email a signed consent form to mcneelymedicalclinic@gmail.com to begin the process. The fee for transfer of medical records is $25.00-$40.00, depending on the size of your chart. If you have any questions about your medical records, please feel free to email to mcneelymedicalclinic@gmail.com or leave a voicemail at 613-253-2233. Note that it may take up to 1 week for a response.
If you are another healthcare provider, or third party agency requiring assistance please feel free to leave a voicemail at 613-253-2233 or email to mcneelymedicalclinic@gmail.com.

Please do not call McNeely Medical Clinic for transfer of care to Westend FamilyCare Clinic, but contact the Westend FamilyCare Clinic directly at 613-599-3321.

Should you wish to not enrol with your physician at Westend FamilyCare Clinic, but will seek care elsewhere and require a copy of your medical records, McNeely Medical Clinic can either transfer your medical records to your new provider on written request from that provider, or can provide you with an encrypted CD containing lab and diagnostic testing reports, specialist and hospital reports, immunization records and treatment history. The fee for a copy or transfer of medical records is $25.00-$40.00, depending on the size of your chart. Please contact the clinic directly if you wish to obtain a copy of your records be mailed to you directly (by email: mcneelymedicalclinic@gmail.com or voicemail at 613-253-2233).

If you decide not to join the new practice location, we encourage you to seek continuity of care by contacting Health Care Connect at 1-800-445-1822 for information on other family physicians in the area.

NOTICE TO DR. AL-HINDAWI & DR. TAI PATIENTS:

We have had the privilege of serving the community of Carleton Place and surrounding areas for many years. We are sorry to inform you that our practices at McNeely Medical Clinic will be closing as of December 31, 2022. McNeely Medical Clinic has worked very hard over the past years to advocate for rural healthcare, and we share with clinic management and staff the great disappointment that it has become necessary to close our clinic.

Dr Al-Hindawi and Dr Tai will be moving to Westend FamilyCare Clinic Family Health Team at 80 Michael Cowpland Drive in Kanata (as of January 9, 2023 for Dr Al-Hindawi and February 13, 2023 for Dr Tai). Patients who wish to continue with Dr Al-Hindawi and Dr Tai are welcomed to have their medical records and registration transferred to the Westend FamilyCare Clinic (request must be received until April 9, 2023 for Dr Al-Hindawi and May 14, 2023 for Dr Tai). After these dates, we cannot guarantee you a place in our new practice. Patients can transfer their records to the Westend FamilyCare Clinic, free of charge, by calling ***613-599-3321*** starting from Dec 1, 2022 for Dr Al-Hindawi’s patients, and from Jan 1, 2023 for Dr. Tai’s patients.

Please do not call McNeely Medical Clinic for transfer of care to Westend FamilyCare Clinic, but until December 22nd you may continue to call 613-253-2233 to schedule appointments at McNeely Medical Clinic for any semi-urgent issues. After this, please contact Westend FamilyCare Clinic to schedule appointments.

***If you are going to run out of your medications in the next 120 days, we advise that you book an appointment for prescription refills before the closing date of the clinic. Given the expected high demand on appointments in the next two months and the logistical limitations of changing the practice, going forward we will not be able to initiate referrals for non-urgent consults/investigations unless it is time sensitive.***

As mentioned above, this was a very hard decision that was imposed on us by multiple external factors. We are sad to have to close this clinic, but are grateful to be able to offer that our patients continue with us if they wish to do so.

For all the patients who are unable to stay with us, we want to tell you that we will miss you and that we enjoyed being your family physicians. We wish you all the best.
