Pain can tell us there may be a threat to our bodies. Thirst can tell us we have to drink. But, what does stiffness tell us? At an intuitive level, the feeling of stiffness would lead us to assume we HAVE stiff joints… right?...
Thanks to some awesome research, we’ve gained some more clarification on this universal bodily feeling.
Three questions were asked: Does FEELING stiff equate to BEING stiff? Does stiffness provide a protective mechanism for a body in pain? Last, can the feeling of stiffness be influenced by other sensory cues (like the sound of ‘stiffness’ or ‘creakiness’)?
In short, those who perceived feelings of stiffness did not actually have more measured ‘tissue stiffness’ than the average person. And those who had pain and stiffness perceived forces on their bodies better than the average person. Last, feelings of stiffness were actually increased or decreased by an external sound of creaky door.
TAKE HOME: The feeling of stiffness does NOT mean that you actually are stiff or have poor joint health and seems it is more of a protective, potentially learned experience in order to reduce movement and prevent re-injury.

Pain protects us and warns us about impending tissue damage or threat to the body. But like other behaviours (yes, pain is a behaviour), pain is LEARNED, and can therefore exist without any real threat, just like Pavlov’s dog can salivate without dangling a nice piece of meat in front of it.
Many experiences can influence how someone perceives pain and how impactful it can become on their life. One of the greatest factors in this is FEAR/ANXIETY OF HARM. While some may feel pain and not think twice about it, others may be instantly stricken down with fear and worry because they, based on past experience, believe they are in immediate danger.
An enlightening study (Wiech et al. 2010) was completed on a group of healthy subjects. These subjects had a laser shone on their feet in different areas. They were told that some areas were more sensitive (not actually though) and that those areas were going to be closely monitored (to promote fear/worry). Following this, they were ‘zapped’ in several areas of the foot with a stimulus which is typically painful half the time and not the other half.
What they found was subjects experienced significantly MORE PAIN and anxiety when ‘zapped’ in areas they were told were more sensitive/more worrisome, even though the stimulus was always the same!
In other words, pain was created based solely on the IDEA that there was an impending threat or reason to worry. This gives major insight into understanding why some experience pain in ways others may not.

(Let's call her) Maria is a lovely lady who has come down with a nasty case of Achilles Tendinopathy. That thick part at the back of her heel where her calf muscles attach to the foot has become swollen and painful leaving her unable to walk for longer than 5 minutes at a time.
Maria has had some great response to acupuncture in the past for other issues so we used that to our advantage along with instrument-assisted soft tissue release to address some of the scar tissue that had formed.
On top of that, Maria is diligent in completing her gradual loading exercises each day. After a few visits, she is up to about 30 minutes of walking until discomfort.
