✨Maid Doesn’t Drown✨

In Canada, and some parts of the United States where it’s been legalized, Medical Assistance in Dying, often called MAiD, is a legal and carefully regulated medical process. For many people, it brings a sense of peace, relief, and autonomy after long periods of illness, pain, or loss of quality of life.

When someone is nearing death, whether through MAiD or natural dying, there are physical changes that can be difficult for loved ones to witness if they are unfamiliar with the dying process.

One of the most misunderstood changes is the sound sometimes heard in the chest or throat. This is often referred to as terminal secretions, or what many people call the death rattle.

This sound does not mean the person is drowning or suffocating. As the body begins to shut down, the brain no longer signals the body to swallow in the same way. Saliva and natural fluids can collect in the throat. Because the person is usually deeply unconscious at this stage, they are not aware of the sound and are not experiencing distress from it.

The sound is created by air passing over relaxed tissues and pooled secretions. It can sound wet, gurgly, or rattling, which can be very upsetting for those listening, even though the dying person is typically comfortable and unaware. This is a normal and common part of the dying process.

This is NOT caused from MAiD medication.

It is important to understand that near death, breathing patterns change naturally. Breaths may become irregular, shallow, or spaced far apart. Long pauses can occur. This does not mean the person is struggling. It is the body gradually conserving energy as systems slow and prepare to stop.

For families and loved ones, witnessing these changes can be emotionally intense. Education helps remove fear. Understanding what is happening in the body allows people to remain grounded, present, and compassionate rather than panicked.

When we understand what we are seeing and hearing, we are better able to meet that moment with love rather than fear.

Talking about death isn’t easy, but avoiding the conversation can leave families overwhelmed when the time comes. Starting a discussion about end-of-life wishes allows you to honour your loved one’s choices and provide them with peace of mind.

Here are a few tips to begin:

💜 Start with love – Let them know you want to understand their wishes.
💜 Listen without judgment – This is about their needs and feelings.
💜 Focus on values – Ask what matters most to them as they transition.
💜 Take your time – These conversations don’t have to happen all at once.
