La contre-pulsion externe améliorée (Enhanced External Counterpulsation, EECP) est un dispositif d'assistance circulatoire mécanique sûr, non invasif et économique. Son mécanisme consiste principalement à envelopper les cuisses, les mollets et les fesses du patient dans des manchons gonflables. L'appareil utilise un système électronique pour détecter l'onde R de l'électrocardiogramme (ECG) et calcule les phases systolique et diastolique à l'aide d'un ordinateur. Il émet ensuite des commandes correspondantes pour gonfler et dégonfler les manchons au cours de ces phases. Pendant la diastole, les manchons sont gonflés pour augmenter la pression diastolique, améliorant ainsi la perfusion sanguine et le retour veineux. Pendant la systole, les manchons sont dégonflés, permettant aux artères des membres inférieurs de se relâcher et d'accueillir le flux sanguin provenant de l'aorte, ce qui réduit la pression systolique et diminue la postcharge cardiaque. L'application de la contre-pulsion externe améliorée (EECP) après une intervention coronarienne percutanée (ICP) peut efficacement améliorer la baisse de la pression de perfusion myocardique causée par l'ischémie, renforcer l'hémodynamique et la fonction endothéliale vasculaire, et promouvoir la circulation collatérale coronarienne, prévenant ainsi la resténose intra-stent.
