Meniscal Root tear (4; final post )

Meniscal root tear equals total menisctomy
So it's very important to diagnose and manage it early to prevent accelerated OA of the knee

Conservative treatment
Root repair, HTO or combination of both

This depends on
Patient age, activity, degree of arthritis and varus .. etc

Post operative protocol

Non weight bearing and flexion up to 90° in the first 6 weeks

Meniscal root tear (3):

Truncation sign:

Absent or faint of part of meniscus at its root attachment
appeared in coronal MRI ( posterior coronal cuts near PCL tibial attachment )

Meniscal root tear (2):
===================
Extrusion sign
Displacement of the meniscus beyond the tibial plateau margin, usually more than 3 mm on coronal MRI

This displacement occurs because a root tear disrupts the hoop stress mechanism, so the meniscus can no longer stay in place and bulges outward.

كتفي اول مره يتخلع ... لازم اعمل عمليه ولا لا ؟؟

خلع الكتف اكتر من نوع .. أكثرهم شهره هو الخلع الامامي
بعد ما الكتف يتخلع لازم اعمل اشعه عاديه واشعه رنين و(أوقات مقطعيه) عشان اقدر احدد نوع الاصابه اللي الخلع عملها في الكتف .

نعمل عمليه ولا لا ؟؟
ده مش قرار ثابت لكل المرضي .
في حاجات كتير جدا بتحدد القرار ده .
زي فحص المريض .. السن .. الليونه الزايده في الاربطه .. بيلعب رياضه ولا لا ونوع الرياضه اللي بيلعبها .. الاصابه اللي حصلت في الكتف بسبب الخلع .
في مرضي لازم نعملهم عمليه بعد اول مره خلع .. وفى مرضي لا .

فالقرار الصح اني لازم اكشف والافضل اروح لدكتور متخصص في إصابات ومناظير الكتف وهو يحدد هتحتاج عمليه ولا لا .

In direct sign of ACL tear (3) = anterior tibial translocation (subluxation)

من العلامات الغير مباشره لقطع الرباط الصليبي الأمامي هي حركه عظمة الساق الي الامام مقارنه بعظمه الفخذ لانه هو المسؤول الأساسي عن منع هذا الحركة

Anterior translocation of tibia 7 mm or more in relation to femur is an indirect sign of ACL tear
Appear in sagittal MRI knee of lateral compartment

Frozen shoulder (1)
تيبس الكتف هو فقد او محدوديه بحركه الكتف خصوصا مع مرضي السكر والغدة الدرقية
التشخيص غالبا بيتم بالفحص الاكلينيكي
النسبة الأكبر بتخف بالعلاج التحفظي زي العلاج الطبيعي و حقن الكورتيزون
لو فشل العلاج التحفظي بنلجأ للعمليه
وهي عمليه منظار جراحي لفك التيبس أو التحريك تحت مخدر عام

Frozen shoulder is loss of ROM esp in diabetic and thyroid patients
It is a clinical diagnosis and usually treated by conservative measures like intrarticular steroid injection and physiotherapy
Operative treatment, if failed conservative, include Arthroscopic capsular release or MUGA

خلع عظمه الرضفة (الصابونة) عادة ينتج عنه قطع في أحد الاربطه الاساسية المسؤولة عن ثباتها
اعاده بناء هذا الرباط يستلزم أخذ رقعة وترية وتثبيتها بمكان محدد بالركبة يتم تحديدة بدقة تحت جهاز الأشعة
بالعمليات

Patellar dislocation usually cause tear of MPFL
If Reconstruction of MPFL is indicated, SCHOTTLE point must be detected under image
In dead lateral x-ray knee .. the point is just anterior to posterior femoral line and just proximal to the level posterior point of the Blumensaat line (red line)

Indirect sign of ACL tear (2) = segond fracture

من احد العلامات الغير مباشرة لقطع الرباط الصليبي الأمامي (٢) هو كسر سيجوند ويكون عباره عن كسر نزعي بأحد اربطه الركبة ويظهر في الاشعه العادية والمقطعية

Segond fracture = avulsion fracture of anterolateral ligament (ALL) which is attached to gerdy tubercle
Appear in x-ray and CT knee

اصابه بالكتف لا يتم تشخيصها بسهوله
تؤدي إلي عدم استقرار الكتف والخلع المتكرر له

Diagnosis :
Need high suspicion
MRI in recent trauma
MR Arthogram if injury > week

J sign is pathognomonic for diagnosis

Indirect sign of ACL tear (1) = bone marrow contusion

من احد العلامات الغير مباشرة لقطع الرباط الصليبي الأمامي (١) هو ظهور كدمات بالجزء الخارجي من الركبة في الرنين المغناطيسي

In MRI knee (sagittal view) :The presence of bone contusions in the lateral compartment is a positive predictive value in determining ACL injury.
