Early management of lateral ankle sprains - Don’t ignore the simple

While functional rehabilitation has traditionally been emphasised, and should be followed for grade 1 sprains, there are important considerations to keep in mind when dealing with grade 2 & 3 due to the high recurrence rates of LAS, the development of chronic ankle instability (CAI), and the potential for ankle osteoarthritis (OA)

🛑Stop telling patients with lateral hip pain to stretch! ✋

One of the leading causes of lateral hip pain is Gluteal Tendinopathy. This is an insertional tendinopathy and is aggravated by stretching movements that work into adduction, especially if combined with flexion or rotation.

The classic piriformis stretch or pigeon pose are often suggested for patients with symptoms in this region but actually tend to make things worse!

What if the patient likes to stretch or enjoys yoga? It’s not harmful but it’s best to reduce the intensity and duration of the stretch or focus on other areas, especially when symptoms are irritable.

As things improve people can gradually return to their goal activities (including stretches) and progress based on symptom response. This is important as we’re not suggesting long-term avoidance just short-term activity modifications .

How prevalent is patellar tendinopathy? ( aka jumpers knee)

Patellar tendinopathy is a condition resulting from excessive use, and it often develops gradually over time. Research indicates that the prevalence of this condition is notably higher in male athletes compared to female athletes.

Among the sports studied, basketball players had the highest incidence of patellar tendinopathy, with 36% of athletes in this sport experiencing this condition. The study by Aaron Schwartz and colleagues examined various sports, including basketball, netball, cricket, and Australian football, to assess the prevalence of patellar tendinopathy among athletes.

The intricate relationship between fascia and muscular discomfort is particularly evident in issues related to the upper trapezius muscle.

Fascia, a dense, fibrous connective tissue, envelopes muscles throughout the body, providing support and protection while facilitating movement. However, when this fascia becomes dysfunctional, either through tightness, adhesions, or imbalances, it can significantly impact the well-being of the muscles it surrounds.

In the context of the upper trapezius, which spans the neck, shoulders, and upper back, fascial restrictions can lead to a host of issues.

The upper trapezius is pivotal in various movements and postural stability, but when its fascia is impaired, it can cause increased muscle tension, pain, and reduced range of motion. This is often characterized by stiffness and discomfort in the neck and shoulder area.

Factors contributing to fascial problems in the upper trapezius include poor ergonomic practices, sustained postural strain, and repetitive motion activities. These factors can lead to the fascia becoming less pliable and more prone to forming adhesions, which can bind to muscles and other tissues, restricting movement and causing pain.

To mitigate these issues, it’s crucial to focus on interventions that promote fascial health. This includes regular movement, stretching, myofascial release techniques, adequate hydration, and potentially professional therapeutic interventions like massage, osteopathy or physiotherapy. Addressing fascial health is essential for maintaining the functionality and comfort of the upper trapezius muscle, thus ensuring better overall musculoskeletal health.

Stress is a common trigger of tension-type headaches and migraine. It can also trigger other types of headaches or make them worse. Stress is a particularly common headache trigger in adults. Psychological stress is actually cited as one of the most prominent contributors to headaches, in general. In 2017, a systematic review found that excessive worry and tension likely play a role in migraine episodes.

American Cancer Society screening recommendations for women at average breast cancer risk:

🎀For screening purposes, a woman is considered to be at average risk if she doesn’t have a personal history of breast cancer, a strong family history of breast cancer, or a genetic mutation known to increase risk of breast cancer (such as in a BRCA gene), and has not had chest radiation therapy before the age of 30.
🎀Women between 40 and 44 have the option to start screening with a mammogram every year.
🎀Women 45 to 54 should get mammograms every year.
🎀Women 55 and older can switch to a mammogram every other year, or they can choose to continue yearly mammograms.
🎀Screening should continue as long as a woman is in good health and is expected to live at least 10 more years.
🎀All women should understand what to expect when getting a mammogram for breast cancer screening – what the test can and cannot do.

American Cancer Society screening recommendations for women at high risk:
🎀Women who are at high risk for breast cancer based on certain factors should get a breast MRI and a mammogram every year, typically starting at age 30. This includes women who:
🎀Have a lifetime risk of breast cancer of about 20% to 25% or greater, according to risk assessment tools that are based mainly on family history
🎀Have a known BRCA1 or BRCA2 gene mutation (based on having had genetic testing)
🎀Have a first-degree relative (parent, brother, sister, or child) with a BRCA1 or BRCA2 gene mutation, and have not had genetic testing themselves
🎀Had radiation therapy to the chest when they were between the ages of 10 and 30 years
🎀Have Li-Fraumeni syndrome, Cowden syndrome, or Bannayan-Riley-Ruvalcaba syndrome, or have first-degree relatives with one of these syndromes

PHYSICAL ACTIVITY and CANCER (NATIONAL CANCER INSTITUTE)
❗❗Evidence linking higher physical activity to lower cancer risk comes mainly from observational studies, in which individuals report on their physical activity and are followed for years for diagnoses of cancer. Although observational studies cannot prove a causal relationship, when studies in different populations have similar results and when a possible mechanism for a causal relationship exists, this provides evidence of a causal connection.
✔There is strong evidence that higher levels of physical activity are linked to lower risk of several types of cancer.
🛑Exercise has many biological effects on the body, some of which have been proposed to explain associations with specific cancers. These include:
👉Lowering the levels of sex hormones, such as estrogen, and growth factors that have been associated with cancer development and progression (20) [breast, colon]
👉Preventing high blood levels of insulin, which has been linked to cancer development and progression (20) [breast, colon]
👉Reducing inflammation
👉Improving immune system function
👉Altering the metabolism of bile acids, decreasing exposure of the gastrointestinal tract to these suspected carcinogens (21, 22) [colon]
👉Reducing the time it takes for food to travel through the digestive system, which decreases gastrointestinal tract exposure to possible carcinogens [colon]
❗Helping to prevent obesity, which is a risk factor for many cancers

❗❗OBESITY - A MODERN TIME PANDEMIC
⚠Rates of overweight and obesity have almost TRIPLED since the 1960s.
🔺Overweight/obesity is known to increase the risk of at least 13 different types of cancer: post-menopausal breast cancer, colorectal cancer, kidney (renal cell) cancer, endometrial cancer, thyroid cancer, pancreatic cancer, multiple myeloma, liver cancer, ovarian cancer, adenocarcinoma of the esophagus, gastric cardia (upper stomach) cancer, gallbladder cancer, and meningioma.
✔The link between overweight/obesity and cancer risk is complex and varies with the specific type of cancer, but is thought to involve increased insulin and hormone levels and chronic inflammation.
✔While some overweight/obesity-related cancers, such as breast cancer, colorectal cancer, and kidney cancer are common, others are relatively rare. ✔The extent to which being overweight or obese increases the risk of cancer also varies with cancer site.

Studies have shown that up to 50 per cent of adults over 65 experience urinary or faecal incontinence at some point in their lives. It is a common condition that is often under-reported and undertreated.

This week is World Continence Week. To help raise awareness, Lauren Neill APAM MACP, Sarah Myles APAM and Leanne Slater APAM of the APA Women’s, Men’s and Pelvic Health (WMPH) group present five facts about the effects of urinary and faecal incontinence on health and quality of life for older people, and what kinds of treatment and management physiotherapy can offer.
