I’ve been honored to direct the Facial Plastic Surgery track, at the Emirates Rhinology & Otology Congress, for the 14th consecutive year. We had two full days of didactic lectures followed by the Rhinoplasty workshop which included cadaveric dissections and live surgery performed by and myself.

A deformed nose after infection of a dorsal rib graft. We augmented the nasal dorsum using diced ear cartilage and corrected the Pollybeak and the depressed tip used an ear cartilage caudal septal extension graft that we described in 2023. Correcting a previously operated crooked nose is a very challenging procedure. In this revision case, the deviation was corrected using a technique that we described in 2003 in which the perpendicular plate of ethmoid is used to correct the dorsal cartilaginous septum deviation. For the Pollybeak and the droopy tip, we used an ear cartilage caudal septal extension graft that we described in 2023 and to narrow the nostrils and decrease the alar flare, we used the alar base excision technique that we described in 2007. This patient, with thick skin, came asking for a smaller and cuter nose. When doing Reduction rhinoplasty in thick skin, you are walking a tightrope, as you have to carefully balance between the amount of reduction and the degree of definition. If you do too much reduction in thick skin, the patient will end up with a potato nose with total loss of definition and support. Maintaining a strong framework is a key in any rhinoplasty on thick skin noses 👃

To correct the crooked nose, i used the technique, i published over 20 years ago, in which the perpendicular plate of ethmoid is used to correct the dorsal cartilaginous septum deviation.
