In male rhinoplasty the surgeon has to do everything possible to preserve the masculine criteria of the nose. To do that, 1) you have to leave the nose relatively large and avoid any overreduction, 2) you have to leave the nasal dorsum straight and high, 3) you have to avoid excessive tip rotation, 4) keep the Nasolabial angle around 90 degrees. A patient with a severely deviated nose and nasal septum and an overprojected nasal tip. The crooked nose was corrected using the septal scoring and splinting by bony grafts from the perpendicular plat of Ethmoid, a technique we described in 2005. The overprojected nasal tip was corrected using our Alar Setback technique that we described in 2001.

A patient with thick nasal skin, a large droopy tip, and a wide nasal base. In thick nasal skin, the only way to get good long-term definition and narrowing is by increasing tip support and projection. I’ve been honored to direct the Facial Plastic Surgery track, at the Emirates Rhinology & Otology Congress, for the 14th consecutive year. We had two full days of didactic lectures followed by the Rhinoplasty workshop which included cadaveric dissections and live surgery performed by and myself.

A deformed nose after infection of a dorsal rib graft. We augmented the nasal dorsum using diced ear cartilage and corrected the Pollybeak and the depressed tip used an ear cartilage caudal septal extension graft that we described in 2023. Correcting a previously operated crooked nose is a very challenging procedure. In this revision case, the deviation was corrected using a technique that we described in 2003 in which the perpendicular plate of ethmoid is used to correct the dorsal cartilaginous septum deviation. For the Pollybeak and the droopy tip, we used an ear cartilage caudal septal extension graft that we described in 2023 and to narrow the nostrils and decrease the alar flare, we used the alar base excision technique that we described in 2007.
