Dr. Ashraf: "If you have diabetes, breast reconstruction surgery is possible—but only if your blood sugar is very well controlled. We need tight control before, during, and after the operation. If the diabetes is not well managed, we do not recommend proceeding with reconstruction."

“The ideal breast reconstruction, from a plastic surgery perspective, is what we call aesthetic reconstruction. It focuses on two main aspects:
First, the appearance—it should look natural, be aesthetically pleasing, and be well-matched to the other breast and to the overall body.
Second, the function and durability—it should be something that lasts for the patient long-term without problems, allowing her to live her life normally.

The first study we conducted at Kasr El Ainy Hospital on breast reconstruction surgeries was a comparative study between two types of procedures. After that, we had to develop our own technique, tailored to our patients, to achieve the best results with the lowest rate of complications.
After we achieved the best results with the lowest rate of complications, this study was published in the number one worldwide publication for microsurgery - the Micro Surgery Journal. This research was conducted in collaboration with our colleagues at the University of Birmingham in England.

Of course, my favorite place has always been Kasr Al Ainy Faculty of Medicine — it’s home for us. I completed my Bachelor’s, Master’s, and PhD there.
During my doctoral studies, I obtained membership of the Royal College of Surgeons of England.
After that came the journey to Europe, with the goal of earning the European Board certification in Plastic and Reconstructive Surgery.
At the same time, it was important to gain hands-on exposure and become familiar with different schools of thought in plastic and reconstructive surgery, including breast reconstruction procedures.
The journey began in Glasgow, then continued in Germany — specifically Munich and Stuttgart— and finally in Paris, France.

Honored to have contributed as a panelist at the Annual Conference of the Egyptian Society of Plastic and Reconstructive Surgeons (ESPRS), held in collaboration with ICOPLAST.
It was a privilege to engage in scientific discussion alongside respected colleagues from different regions.
We are grateful for the positive feedback from the audience and for the recognition extended by the ESPRS board. Continuous academic exchange remains essential in advancing both reconstructive and aesthetic surgery standards.

English transcript:
Many women who undergo mastectomy are not aware that there are now multiple options available for breast reconstruction.
These include implant-based reconstruction as well as autologous tissue reconstruction, which we often prefer — especially in patients who will receive radiotherapy.
In such cases, we use the lower abdominal tissue — the same tissue typically removed during cosmetic abdominoplasty (tummy tuck) procedures. Instead of discarding it, we transfer it from the abdomen to the chest to reconstruct the breast. The blood vessels are then reconnected using microsurgical techniques.
The technique we have developed and refined involves taking only a very small portion of the abdominal muscle — or, in some cases, preserving the muscle entirely — in order to significantly reduce potential abdominal complications following surgery.
We have published this technique in studies conducted here in Egypt, as well as in recent collaborative research with the University of Birmingham, published in the Journal of Microsurgery, one of the leading journals in the field of microsurgical procedures.
